Alterations Of Serum HsCRP,APN And Carotid Artery Tissue In Periodontitis Rats With Type 2 Diabetes Mellitus After Tooth Extraction

Objective:To observe the alterations of serum high sensitivity C-reactive protein(hsCRP),adiponectin(APN)and carotid artery tissue in chronic periodontitis(CP)or combined with type 2 diabetes mellitus(T2DM)rats after tooth extraction or plus antibiotics.Methods:Forty-nine SD rats were randomly divided into three groups:A(control group),B(CP group)and C(CP+DM group).According to different intervention measures,group B and C were divided into B~0/C~0(natural process group),B~1/C~1(tooth extraction group),B~2/C~2(tooth extraction plus antibiotics group),and seven rats in each group.Twice extraction interventions were executed.The rats in each group were given intraocular canthal vein blood samples at five time points:one week before intervention(T1),one week after the first intervention(T2),one week after the second intervention(T3),three weeks(T4)and five weeks(T5).The serum hsCRP and APN levels were measured by enzyme-linked immunosorbent assay(ELISA)after centrifugation.All rats were sacrificed at the end of the experiment,their carotid artery and its bifurcation were separated about 1cm above and below.The pathological changes were observed under light microscope after HE staining.Results:1?Serological results:The levels of serum hsCRP in group B and C were significantly higher than group A(P<0.05),and increased with time in group B~0 and C~0.At T2 point,all intervention groupsreachedthehighest value in eachgroup(B~1/B~2:1463.214±81.492/1517.427±136.746,C~1/C~2:980.594±89.113/977.315±262.341ng/ml)and significantly higher than those in group A(403.996±25.396 ng/ml)(P<0.001),then began to decline gradually,and reached the lowest value at T5 point,which was significantly lower than the baseline value of each group(P<0.001)(there was no statistical difference in group C~1).Besides in group C~2 was significantly lower than group C~1(P<0.001),and in group B~2was lower than group B~1(no statistical difference).The levels of serum APN in group B~0and C~0 declined with time.At T3 point,all intervention groups reached the lowest value in each group(B~1/B~2:36.971±3.704/36.029±4.179,C~1/C~2:31.157±8.177/33.927±16.972ng/ml)and significantly lower than those in group A(56.586±7.815 ng/ml)(P<0.001),then began to increase gradually,and reached the highest value at T5 point,which was significantly higher than the baseline value of each group(P<0.001)(except group C~1).Besides in group C~2 was significantly higher than group C~1(P<0.001),and in group B~2 was higher than group B~1(no statistical difference).2?Histopathology showed that in group A,the intima was intact,the elastic fibers and smooth muscle cells in the middle membrane were arranged neatly,and the vessel wall was not thickened.In group B~0 and C~0 the intima was incomplete,the elastic fibers in the middle membrane were arranged disorderly and some of them were broken.The smooth muscle cells in the middle membrane were vacuolated and some of them were necrotic(more obvious in group C~0),the vessel wall was thickened.In group B~1,the intima was incomplete,the elastic fibers in the middle membrane were broken,dissolved and disappeared.Calcium deposits were found and the smooth muscle cells were vacuolated,different thickness of the vessel wall was observed.In group C~1,the intima was incomplete,the elastic fibers in the middle membrane were broken,dissolved and even disappeared.Partial plaque were dropped after staining,the smooth muscle cells were vacuolated and thinness of the vessel wall was observed.In group B~2 and C~2,the intima was intact,the elastic fibers in the middle membrane were arranged disorderly and some of them were bifurcated,some of the smooth muscle cells were vacuolated and the vessel wall was not thickened.Conclusion:1?If periodontitis rats with or without type 2 diabetes mellitus is not been treated,the levels of serum hsCRP may continue to develop and APN may continue to decrease due to the continuous development of periodontal inflammation,which lead to increase the lesions of vascular tissue in rats(especially when combined with diabetes mellitus),so hsCRP and APN may participate in the development of vascular lesions.2?Periodontitis rats with or without type 2 diabetes mellitus might aggravate vascular tissue lesions(especially when combined with diabetes mellitus)in a short time after tooth extraction.If a little of antibiotics could be used before and after tooth extraction,the degree of vascular tissue lesions in rats might be greatly reduced and better therapy effect could be achieved.
