Objective To preliminarily understand the etiology,clinical characteristics,drug treatment and efficacy of patients with chronic heart failure in Tibet,so as to provide scientific basis for the prevention and treatment strategies of chronic heart failure in the future,and to collect data for the larger epidemiological investigation of chronic heart failure in Tibet in the futureMethods Continuous in January 1,2016 to December 31,2018 in the Tibet autonomous region people's hospital of cardiology,hospital and patients diagnosed with chronic heart failure,The general epidemiological data,etiology,cardiac function grade,complications,laboratory and instrument examination,main therapeutic drugs and treatment results of the patients were recorded for retrospective analysis and comparisonResults(1)A total of 658 cases of hospitalized patients with chronic heart failure were investigated in this study,including 323 males(49.1%)and 335 females(50.9%).There were 648 Tibetans(98.5%)and 6 Chinese(0.9%).The age ranged from 20 to 95 years old,with an average age of(62.2 ± 14.30)years.The average length of stay was(14.10±5.92)days.(2)The main causes of chronic heart failure patients hospitalized patients with hypertensive heart disease(38.1%),followed by cor pulmonale(14.1%),high grade heart disease(12.0%),rheumatic heart disease(9.1%),DCM(6.9%),congenital heart disease(6.2%),coronary heart disease(4.1%),others(9.5%).(3)The most common complications in patients with chronic heart failure were arrhythmia in 363(55.2%),followed by pulmonary infection in 214(32.5%),hypertension in 133(20.2%),hypothyroidism in 81(12.3%),and diabetes in 50(7.6%).(4)Diuretics,aldosterone receptor antagonists,ACEI/ARB,beta blockers,and antibiotics were the main drugs used during hospitalization in patients with chronic heart failure,with the application rates of 93.5%,78.1%,73.6%,47.9%,and 40.7%.(5)The prognosis of chronic heart failure patients was mainly effective,with the symptoms of heart failure basically disappeared or significantly improved in 97.7%,symptoms did not improve or even worsened in 1.8%,and the inpatient mortality was 0.5%.(6)Ejection fraction decreased heart failure(HFrEF)in 109(23%),ejection fraction medium range heart failure(HFmrEF)in 99(20.1%),ejection fraction retained heart failure(HFpEF)in 270(56.9%).Compared with patients in the HFr EF group,patients in the HFp EF group were more likely to have hypertensive heart disease and pulmonary heart disease,and were more likely to have hypertension and diabetes(P<0.0001).(7)on admission,the composition ratios of grade ?,?,and ? in cardiac function(NYHA classification)were:232(35.3%)in grade ?,362(55%)in grade ?,and 64(9.7%)in grade ?,respectively.The lower the cardiac function,the higher the incidence of arrhythmia and pulmonary infection(P<0.01).The worse cardiac function was the utilization rate of antibiotics,aldosterone receptor antagonists,nitrate esters and digitalis increased,while the utilization rate of calcium antagonist grade antiplatelet aggregation drugs decreased(P<0.01)Conclusions In this area,The hospitalized patients of chronic heart failure i are mainly the elderly.Ejection fraction reserved heart failure(HFpEF)was dominant.The main causes were hypertension heart disease,followed by pulmonary heart disease,gaogenic heart disease and rheumatic heart disease.The main etiological composition of heart failure patients is different from that of other regions in China.Patients with chronic heart failure in this region were mainly treated with diuretics,aldosterone receptor antagonists,ACEI/ARB,beta blockers and antibiotics during their hospitalization.The guidelines for basic adherence to medication are recommended.
