Objective: By detecting the expression levels soluble endoglin(s Eng)in maternal serum and matrix metalloproteinase-14(MMP-14)and endoglin(Eng)in placenta,the study investigated the expression levels of MMP-14,,Eng and s Eng in preeclampsia patients,analyzed the correlation among the three,and investigated the relationship between MMP-14 of key regulator and outcomes of maternal and infant,thus providing new ideas for clinical prediction and diagnosis and treatment.Methods: A total of 64 pregnant women who were delivered in our hospital were selected to investigate the expressions of MMP-14,Eng and s Eng in preeclampsia patients and analyze their correlation.Among selected pregnant women,there were 20 patients with early-onset preeclampsia in the study group(group A),24 patients with late-onset eclampsia in the study group(group B)and 20 cases of normal pregnant women who were in late pregnancy and had a cesarean section in the same period in the control group(group C).In order to further analyze the relationship among MMP-14,Eng and s Eng,patients with early-onset preeclampsia were divided into four groups according to the pre-eclampsia onset gestational week.At gestational weeks of less than 28 weeks(group I),28-30 weeks(excluding 30 weeks,group ?),and 30-32weeks(excluding 32 weeks of group ?),there are 4 cases of early-onset preeclampsia each,additionally 8 cases at 32-34 weeks(excluding 34 weeks,group IV).The expression of MMP-14 and Eng in the placenta was detected semi-quantitatively by immunohistochemistry,and the expression of s Eng in serum was quantitatively determined by ELISA.The analysis was performed using the spss23.0 softwarepackage,and the measurement data were expressed as mean standard deviation.One-way NAOVA analysis was used to compare multiple groups,and SNK-q test was used to further compare the groups.Pearson correlation analysis was used to test the correlation between the two variables.Results: 1.Both MMP-14 and Eng were expressed in the placenta of the normal group and the preeclampsia group,but they were highly expressed in the placenta of patients with preeclampsia,and the expression in patients with early-onset preeclampsia was higher than in patients with late-onset(P< 0.05).2.s Eng was expressed in both the normal group and the study group,but it was highly expressed in the serum of preeclampsia patients,and the expression of early-onset preeclampsia patients was higher than that of late-onset patients(P< 0.05).3.Positive correlation between MMP-14,Eng and s Eng was in both early-onset and late-onset preeclampsia(P< 0.05).4.In early-onset preeclampsia,the expressions of MMP-14,Eng and s Eng were different with the onset of gestational weeks.The differences between the four groups were statistically significant(P< 0.05).However,there was no statistically significant difference between group ? and group ?after a pairwise comparison between the four groups(P> 0.05).5.The neonates in the preeclampsia group were lighter and had longer postoperative hospital stays,and the neonates of the early-onset preeclampsia group was lighter than the late-onset preeclampsia group and the postoperative hospital stay was longer,where the differences were statistically significant(P <0.05);but the expression of D-D dimer was not significantly different between the three groups(P> 0.05).There was a negative correlation between MMP-14 and neonatal weight in the early-onset preeclampsia group,a positive correlation between MMP-14 and the length of hospital stay after surgery(P< 0.05).Conclusions: 1.MMP-14,Eng and s Eng are expressed in the placenta of normal pregnant women and patients with preeclampsia,indicating that all three are important factors during pregnancy.2.The expression levels of MMP-14,Eng and s Eng in patients with early-onset preeclampsia were higher than those in late-onset preeclampsia and normal groups,indicating that early-onset preeclampsia occurs earlier,is more severe,and has higher expression.3.There was a positive correlation between the expressions of MMP-14 and Eng and between MMP-14 and s Eng in the early-onset and late-onset eclampsia groups.MMP-14 can regulate the release of s Eng.All three participate in the pathophysiology of preeclampsia.4.The expression levels of MMP-14,Eng and s Eng are higher in patients with early-onset preeclampsia,and the earlier the onset of pregnancy,the higher the level of MMP-14,Eng in the placenta and the level of s Eng in the serum.But there was no significant difference in the expression levels of the three between the group ?and ?,that is,the expression levels of the three increased in the onset of gestational weeks from 28 to 32 weeks but there was no statistical difference,indicating that 28 and 32 weeks are the key nodes for the treatment.5.Early-onset preeclampsia group had worse maternal and infant outcomes.The neonates of the early-onset preeclampsia group was lighter than the late-onset preeclampsia group and the postoperative hospital stay was longer,and there was a negative correlation between MMP-14 and neonatal weight in the early-onset preeclampsia group and a positive correlation between MMP-14 and the length of hospital stay after surgery as well,indicating MMP-14 may be a key regulator of improving maternal and infant outcomes...
