Objective:Comparing the effect of acupuncture on the pharyngeal acupoint on the affected side and acupuncture on the bilateral pharyngeal acupoint to treat true bulbar paralysis after cerebral infarction,it provides a more optimized treatment plan for clinical treatment.Methods:64 patients with true bulbar palsy after cerebral infarction were divided into the affected group and the bilateral group by random number table method,32 cases in each group, including 1 case in each group,62 cases were actually counted,31 cases in each group. On the basis of conventional drugs and electroacupuncture treatment, the two groups of patients acupuncture the swallowing, pronunciation and treatment of reflux points on the affected side;the bilateral group acupuncture the bilateral swallowing, pronunciation and treatment of reflux points.Acupuncture once a day for 6 days a week, rest for1 d,and evaluate the clinical efficacy after 21 days. The Watian drinking water test and Frenchay dysphonia evaluation scale were used to evaluate the swallowing and dysphonia of the two groups of patients before and after treatment. Results:After 21 days of treatment, the experimental grades of Watian drinking water in the two groups were lower than before treatment(all P<0.01),and the grade reduction of the affected side group was better than that of the bilateral group(P<0.01);Frenchay articulation of the two groups of patients The scores of the Disorder Evaluation Scale were all higher than before treatment(all P<0.01),and the scores of the affected side group were higher than those of the bilateral group(P<0.05).The recovery rate and total effective rate of swallowing function in the affected group were 87.10%(27/31)and 93.55%(29/31)respectively;the recovery rate and total effective rate in the bilateral group were 67.74%(21/31),80.65%(25/31).The recovery rate and totaleffective rate of the articulation function in the affected group were 74.19%(23/31)and 96.77%(30/31)respectively;the recovery rate and the total effective rate of the bilateral group were 45.16%(14/31),respectively,87.10%(27/31).In the treatment of dysphagia and dysarthria, the cure rate and total effective rate of the affected group were higher than those of the bilateral group(both P<0.05).The results of subgroup analysis showed that in the acute phase and the recovery phase of the Watian drinking water experiment, the grade reduction of the affected side group was better than that of the bilateral group(all P<0.05);the Frenchay dysarthria evaluation scale score after the acute phase treatment, the affected side There was no significant difference between the two-sided group and the two-sided group(P>0.05),and the score of the affected side group during the recovery period was higher than that of the bilateral group(P<0.05).Conclusions:1.Both acupuncture on the affected side and acupuncture on both sides of the pharyngeal acupoints can improve the dysphagia and dysarthria of true bulbar paralysis after cerebral infarction, and the effect of acupuncture on the affected side is better than acupuncture on both sides.2.Acupuncture at the pharyngeal acupoints on the affected side for the treatment of dysphagia after true cerebral palsy after cerebral infarction has better curative effect than bilateral in the acute phase and recovery phase.3.Acupuncture at the pharyngeal acupoints on the affected side to treat the dysarthria of true bulbar paralysis after cerebral infarction has no difference in curative effect between bilateral and acute sides, and the recovery period is better than bilateral.
