Objective1.To investigate the clinical effect of the treatment of scapulohumeral periarthritis by the combination of thumbtak intradermal needling and mobilization with movement, and to provide scientific basis for the clinical treatment of scapulohumeral periart hritis.2.Explore an effective, economical and safe treatment of frozen shoulder,to reduce the pain of patients and improve their quality of life, and provide a scientific basis for clinical treatment of frozen shoulder.MethodsFrom January 2019 to December 2019,patients with scapulohumeral periarthritis who meet the inclusion and exclusion criteria in the outpatient and inpatient department of orthopedic injury Department of the First Affiliated Hospital of Heilongjiang University of traditional Chinese medicine were selected as the study objects,Using the random number table randomly divided into control and observation groups. The control group was treated with mobilization with movement, and the treatment group was treated with thumbtak intradermal needling and mobilization with movement,Once a day,7 days is a course of treatment, intervention is 2 courses, two groups of patie nts are instructed to perform functional exercise after each treatment.Visual Analogue Scale(VAS)?Constant-Murley Score(CMS)?Activities of Daily Living Barthel Index(ADL-BI)were used to evaluate the intervention of 1 treatment course,2 treatment courses, and 2 treatment courses after the end of 1 Months of local pain, shoulder joint function and activities of daily living activities were evaluated at the end of 2 courses of treatment.Safety evaluation criteria were used to evaluate the occurrence of daily adverse reactions in the two groups during intervention.Results1.A total of 70 cases were collected in this study, the observation group included 35 patients,of which 2 patients fell off due to personal reasons and did not return to hospital for follow-up;the control group included 35 patients,of which 1 patient was excluded because of self-administration of analgesic and anti-inflammatory drugs,2 patients fell off due to personal reasons and did not return to the hospital for follow-up.No adverse reactions occurred in both groups. Finally, the observation group included 33 cases of statistical analysis and 32 cases of the control group.2.The local pain was compared between the two groups. There was a statistically significant difference between the two groups of patients with 1 course of intervent ion,2 courses of intervention, and 1 month follow-up after the 2 courses of VAS score(Z=-6.404,P=0.000;Z=-4.211,P=0.000;Z=-3.095,P=0.002).3.Two groups of patients with intervention pain degree, daily life;functional range of motion of shoulder joint and 7 days of shoulder joint function CMS total score difference was statistically significant(Z=-4.082,P=0.000;Z=-3.524,P=0.000;Z=-1.949,P=0.049;Z=-2.874,P=0.004),and there was no significant difference in muscle strength(Z=-0.440,P=0.660).Two groups of patients with intervention in 14 days pain degree, daily life, and the function of shoulder joint mobility CMS shoulder function score difference was statistically significant(Z=-4.159,P=0.000;Z=-4.515,P=0.000;Z=-3.716,P=0.051;Z=-4.213,P=0.000),and there was no statistically significant difference in terms of strength(Z=-0.032,P=0.975).Two period of treatment in both groups after one month follow-up in the degree of pain, daily life, and the function of shoulder joint mobility CMS shoulder function score difference was statistically significant(Z=-4.458,P=0.000;Z=-5.308,P=0.000;Z=-3.143,P=0.002;Z=-4.246,P=0.000),and there was no statistically significant difference in terms of strength(Z=-0.032,P=0.975).4.Two groups of patients with intervention for 7 days and 14 days,1 months follow-up after 2 courses of daily life activities ability of ADL-BI index difference had statistical significance(Z=-2.016,P=0.044;Z=-3.280,P=0.001;Z=-2.900,P=0.004).5.After 2 courses of treatment, the total treatment efficiency of the two groups of patients was compared, the control group was 84.4%,the treatment group was 97.0%,the difference between the groups was statistically significant(Z=-2.140,P=0.032).Conclusions1.The combination of thumbtak intradermal needling combined with mobilization with movement is superior to simple mobilization with movement in the treatment ofperiarthritis of shoulder, the reduction of local pain, the improvement of shoulder joint function and the ability of daily life.2.The combination of thumbtak intradermal needling and mobilization with movement can significantly relieve local pain in patients with periarthritis of the shoulders, improve shoulder joint function and daily life activities, and improve the overall efficiency of treatment;and the treatment is easy to carry out,has obvious efficacy, high safety, and is painless. Moreover, the therapy is easy to operate, effective, safe, painless and highly accepted by patients, which has clinical significance.
