| Objective:By comparing the clinical treatment difference between Acupotomy combined with manipulation,Acupotomy alone and manipulation alone in the treatment of lumbar facet synovial incarceration.To explore the pathogenesis of lumbar facet joint synovial incarceration,and provide objective,real and reliable data for the future clinical research.To analyze the clinical effect and feasibility of acupuncture knife combined with manipulation in the treatment of lumbar facet synovial incarceration.Methods:A total of 96 patients who were diagnosed with synovial incarceration of lumbar facet joints were collected from the orthopaedics clinic of First Affiliated Hospital of Heilongjiang University Of Chinese Medicine in 01,2018,?2020,01.According to the method of random number table,32 cases were divided into acupuncture knife group,manipulation group and acupuncture knife combined manipulation treatment group.The acupuncture knife group and the manipulation group were given a single treatment,while the treatment group was given acupuncture knife treatment and manipulation treatment at the same time.The treatment outcome was scored by Number rate scale(NRS),Oswestry disability index(ODI)score and lumbar activity(LMS)score as clinical observation indicators.Spss22.0 software was used to analyze the data and compare the component differences of the three groups before and after treatment.Result:1.three groups of patients at 1 weeks,2 weeks after treatment,NRS scores were decreased to varying degrees,three groups of patients with pain symptoms were significantly improved,the treatment group pain improved most significantly(p<0.01),there was statistical significance,there was a very significant difference,indicating that the treatment group curative effect is better than Acupotomy group,manipulation group.2.The ODI of the three groups decreased in different degrees at 1 week and 2 weeks after treatment,and the lumbar activity function of the three groups was improved significantly,the most obvious improvement was in the treatment group(P<0.01),which was statistically significant,with a very significant difference,indicating that the therapeutic effect of the treatment group was better than that of the needle knife group and the manipulation group3.The activity of lumbar vertebrae(forward flexion,back extension,left,right flexion and left and right rotation)in the three groups increased in different degrees at the first and second weeks of treatment,and the activity in all directions was significantly improved,especially in the treatment group(P<0.01)4.Three groups of patients treatment methods and excellent and good rates:1 weeks after treatment,the overall effective rate of the Acupotomy group was 81.20%,the overall effective rate of the manipulation group was 71.90%,and the overall effective rate of the treatment group was 93.70%.At 2 weeks,the overall effective rate of the Acupotomy group was 87.50%,the overall effective rate of the manipulation group was 93.70%,and the total effective rate of the treatment group was 100%.After statistical Ridit analysis,p<0.05 was statistically significant.There was significant difference between the three groups.The excellent and good rate of the treatment group was significantly higher than that of the Acupotomy group and the manipulation groupConclusion:1.The acupuncture knife group combined with manipulation can effectively relieve the pain of patients with lumbar facet synovial incarceration,recover the lumbar activity including forward flexion,back extension,left and right flexion and left and right rotation activity,and improve the quality of life2.The clinical effect of the treatment group was better than that of the Acupotomy group and the manipulation group3.The treatment of lumbar facet synovium incarceration with Acupotomy combined with manipulation has the advantages of small injury,easy operation,less pain,low cost,high safety and exact effect,which is worthy of clinical application. |
