| Background: Infectious diseases have always been a major public health problem that endangers human health and seriously threatens human health.Since entering 2000,common infectious diseases such as AIDS,tuberculosis,and hepatitis virus have revived,and new infectious diseases have threatened human health from time to time: from the outbreak of domestic SARS in 2003 to the spread of foreign H1N1 in Mexico,Middle East respiratory syndrome and the Ebola epidemic,from the concentrated outbreak of major infectious diseases such as variola virus in the past,to the recent emergence of new coronavirus infections in many places in China,and the trend of global spread.The impact is getting bigger,and the degree of harm is gradually deepening.As there is no quantitative evaluation of the overall situation of the infectious disease management operation mechanism in China,this study studies the infectious disease management operation mechanism from the macro policy level,from the four aspects of management and monitoring,planning and evaluation,financing and compensation,coordination and incentive mechanism,to open a new way for comprehensive prevention and control of infectious diseases.Objective : Analyzing the current status of the construction of infectious disease management operation mechanism in Zhejiang and Shanghai,and trying to quantitatively sort out the causes of the imperfect infectious disease management operation mechanism in Zhejiang and Shanghai.Based on the qualitative interpretation of policy documents and the verification of quantitative indicators,the two Optimization strategies and suggestions for the management operation mechanism of the field of infectious diseases.Method:Taking the macro model of the health system and scientific procedures for policy development as the main research methods,basing on the ideas of normative gap analysis and comparative analysis.Through the horizontal and vertical analysis and quantitative evaluation of the current status of the management operation mechanism of Zhejiang and Shanghai,the ratio to the optimal field and appropriate standards,clarifiing the advantages and disadvantages of the management operation mechanism of Zhejiang and Shanghai.Using SPSS16.0 to organize and analyze the data: using correlation and regression analysis to verify the relationship between the evaluation results and public health results.Results:(1)Quantitative evaluation of the soundness of the operation mechanism of infectious disease management in Zhejiang and ShanghaiQuantitative evaluation of the soundness of the management and monitoring mechanism:comprehensively quantified that the soundness of the management and monitoring mechanism in Zhejiang and Shanghai is basically the same,38.0% and 39.7%respectively,and the ratios to the appropriate standards are 44.7% and 46.7%respectively There was a significant negative correlation between the soundness of management and monitoring mechanism and incidence of Class A and B infectious diseases(P<0.01),and the degree of equation interpretation was 25.3% and 64.4%,respectively.Quantitative evaluation of the soundness of the planning and evaluation mechanism:Comprehensively quantifying the soundness of the infectious disease plan and evaluation mechanism in Zhejiang Province is 49.3%,which is lower than Shanghai(59.0%),and the ratios to the appropriate standards are 58.2% and 69.4%.There was a significant negative correlation between the soundness of the planning and evaluation mechanism of the two places and incidence of Class A and B infectious diseases(P<0.01),and the degree of equation interpretation was 32.1% and 81.0%,respectively.Quantitative evaluation of the soundness of financing and compensation mechanism:Comprehensively quantified that the soundness of financing and compensation mechanism in Zhejiang and Shanghai is 66.3%;the soundness of the financing and compensation mechanism in the two places and incidence of Class A and B infectious diseases are significantly negative correlation(P<0.01),the degree of interpretation of the equation was 23.3% and 54.0%,respectively.Quantitative evaluation of the soundness of the coordination and incentive mechanism:comprehensively quantified that the soundness of the coordination and incentive mechanism in Zhejiang Province is significantly lower than that in Shanghai(24.8% vs61.1%),and the ratios to appropriate standards are 40.6% and 71.9%,There was a significant negative correlation between the soundness of the coordination and incentive mechanism between the two places and incidence of Class A and B infectious diseases(P<0.01),and the degree of interpretation of the equations was 30.1% and 46.1%.(2)Quantitative evaluation of the soundness and health outcomes of infectious disease management mechanism in Zhejiang and ShanghaiBased on comprehensive quantification,the comprehensive degree of infectious disease management and operation mechanism in Zhejiang and Shanghai is 44.8% and 55.1%,respectively.Shanghai is slightly better than Zhejiang.With the improvement of the infectious disease management and operation mechanism in Shanghai and Shanghai,the health outcomes of the two places have gradually improved.There is a significant negative correlation between the improvement of the management and operation mechanism and incidence of Class A and B infectious diseases in the two places(P<0.01).The degree of interpretation of the equation is 31.3% and 65.4%,respectively.Conclusion:(1)Development and construction of infectious disease management operating mechanism in Zhejiang and ShanghaiThe responsibilities of the infectious disease-related business departments in Zhejiang and Shanghai have been basically clear,the incentive mechanism has basically covered the relevant parties,and the medium-and long-term planning has basically covered the problem of infectious diseases.In terms of the construction of the coordination mechanism,although the differences between Zhejiang and Shanghai are large,but The two places have gradually attached importance to ensuring the authority of coordination from the legislative level.(2)The construction of infectious disease management operation mechanism in Zhejiang and Shanghai still needs long-term struggleAlthough fund-raising and compensation mechanism have been basically established in the areas of infectious diseases in Zhejiang and Shanghai,the total funding input,growth mechanism,and government dominance have not yet been quantified.In addition,the responsibilities of the strong support departments of the two places still need to be improved,especially the fiscal responsibilities of the two places.The department’s responsibilities still need to be clear,which hinders the implementation of fundraising and incentive mechanism;Zhejiang and Shanghai also lack the setting of quantitative and sensitive indicators. |
