| BackgroundInfectious disease has been perceived again during these years. As the report and management is the most important part of infectious disease control, our goverment has put out a series of documents and has built up "China information system for disease control and prevention" to collect the message of infectious dieases. At the same time, Shanghai medical institutions supported by social capital have been growing up since the year 2000. But we don't hold any data on the situation of the report and management of infectious disease about such institutions in Shanghai up to now. In order to improve the level of infectious disease control in Shanghai it's necessary to take the teins of the report quality and the management instance of infectious diease and to establish a directorial system in Shanghai medical institutions supported by social capital.ObjectiveBased on the situation of basic complexion, the report and management instance in Shanghai medical institutions supported by social capital, this research will explore a mode and several countermeasures for the health administrative authorities to make decision so as to improve the level of infectious disease control in Shanghai.MethodThis research gathered information by complete check and spot check; by quantitative analysis and qualitative analysis; by interview, questionnaire survey and collecting daily data. We collected the message on the basic establishments, carried out the questionnaire survey on doctors and infectious diseases reporters and interviewed the masters in charge of infectious disease report work in the investigated medieal treatment orgainzations. The database was set up by Epidata software and analyzed by Excel and SPSS; Using the method of class attribute to analyze the information collected by interview.Results1. The number of medical institution supported by social capital in Shanghai was 1083 with different class institution, different dimensions and hardware.2. The institutions couldn't offer the documents provided by govement; there was no institution has put up research on infectious disease and the proportion of infectious diseases reporters and directive systems in hospital was higher than clinic and clinique.3. It was hard to use the index of misreport, compeleteness and consistency because of the bad quality of basic register; the timeless (92.72%) and the rereport (1.10%) were worse than public treatment institution.4. The scores of exeicises about infectious diease report and the "Law of the PRC on the Prevention and Treatment of Infectious Diseases" got by doctors and infectious disease reporters were low, those who working in clinique got the lowest scores. The effect of training was terrible. Doctors who work in different section office, with different school degree, different title of technical post and whether attended the training got the same level.5. The medical institutions attached less importance to work on the report and management of infectious disease so as to they couldn't put the documents into effect, and the health administrative agency, the department of disease control and the supervisory department put their heart into monitor and ignore the guidance.Discussion and SuggestionsBy investigating and analyzing the situation of medical institutions supported by social capital in Shanghai, we found that different class institutions had fit out different dimensions and hardware. The index of report quality and the score of exeicises related by infectious diease report and law were low. The effect of training was terrible and the direction and the intendance given by health administrative agency was not equity. The institutions attached little importance to work on the report and the management of infectious diseases.In order to improve the level of infectious disease control in Shanghai, the administrative agency should establish the standard of institution's approvement, create the conditions of infectious disease reporting, adjust the documents which contrapose the medical institutions supported by social capital and put the documents in practice, improve the quality of report and the scores of exercises, strengthen the guidance of document to all of the medical institutions supported by social capital such as dispose the guidance to public medical institutions. Meanwhile, the institutions should also play a role in self-regulation. |
