Objectives:The aim of this study was to investigate the status of sleep quality and heart cerebrovascular diseases(hypertension,coronary heart disease and stroke)in rural elderly aged 60 years and over in Ning'er County of Yunnan Province.To analyze the influence factors of sleep disorder.And to explore the relationship between sleep quality and heart cerebrovascular diseases,so as to provide a theoretical basis for the early intervention of sleep habits,improvement of sleep quality,prevention and control of heart cerebrovascular diseases.Methods:Multistage stratified random sampling method was conducted to select 1700 residents aged 60 years and over from Ning Er County in Yunnan province.Each participant participated in a structured interview,and received physical examination.Information on demographical characteristics,sleep quality status and prevalence of heart cerebrovascular diseases(hypertension,coronary heart disease and stroke)were obtained by a standard questionnaire.The Pittsburgh Sleep Quality Index(PSQI)was applied to assess sleep quality of the elderly.The height,weight,waist circumference,hip circumference and blood pressure were recorded by physical examination.The field survey data were double entered in Epidata3.1 software and analyzed by SPSS23.0 software.Chi-square test was used to compare the difference of composition ratio and rate of different groups,t-test was used to measure the continuity data.The normal distribution data of different age groups were analyzed by one-way ANOVA,and the non-normal distribution data of different age groups were tested by Kruskal Wallis H test.And multivariate logistic regression was applied to analyze the influce factors of sleep disorder and to model the variation in prevalence of sleep disorder and heart cerebrovascular diseases.Statistical significance was based on two-tailed P<0.05.Results:A total of 1,700 rural elderly aged 60 years and older in Ning'er county were investigated and 1629 valid questionnaires were received in our study,representing an overall response rate of 95.82%.Of the 1,629 study participants,688 were male(42.23%)and 941 were female(57.77%).Women had a worse status than men in five domains of subjective sleep quality,sleep latency,habitual sleep efficiency,sleep disturbance and daytime dysfunction,in addition,the total score of PSQI was significantly higher than men(P<0.001).There were differences in sleep latency,sleep duration,sleep efficiency,sleep disorder and the total PSQI score among the elderly of different age groups(P<0.05).The prevalence of sleep disorder,hypertension,coronary heart disease and stroke among the rural elderly? 60 years old in Ning'er County of Yunnan Province were 56.78%,52.85%,8.16%and 8.66%,respectively.The prevalence of sleep disorder was different in different gender,age groups,nationalities,education level,annual average family per capital income and access to medical services(P<0.05).The prevalence of hypertension of the 60?64 age group was lower than other three groups(P<0.001),and the prevalence of coronary heart disease of ?75 age group was higher than other three age groups(P<0.001).The prevalence of stroke in male was significantly higher than female(P<0.05).The prevalence of stroke of the 60?64 age group was lower than other three groups(P<0.001).And the higher family per capita annual income,the higher the prevalence of stroke(P<0.05).The results of multivariate logistic regression analysis in our study indicated that female,high age groups and Ethnic minorities were the risk factors of sleep disorder in the elderly.And higher family per capita annual income and better access to medical services were the protective factors of sleep disorder in the elderly.At the same time,analysis results also demonstrated that sleep disorder was a risk factor of heart cerebrovascular diseases in the elderly.The elderly with sleep disorder were more likely to suffer from hypertension(OR=1.307),coronary heart disease(OR=2.967)and stroke(OR=1.511)than the elderly without sleep disorder,respectively.Conclusions:The older adults aged 60 years and over in Ning'er County of Yunnan Province had a poor sleep quality.And the prevalence of sleep disorder,hypertension,coronary heart disease and stroke were high.Sleep disorder may be a risk factor for these three heart cerebrovascular diseases.Our results suggested that sleep health education should be strengthen for the rural elderly in Ning'er County of Yunnan Province,and early sleep interventions should be implemented,especially for the female,senior elderly,ethnic minorities and the older adults with lower family per capita annual income and poorer access to medical services so as to improve their sleep quality.Furthermore,it is essential to strengthen work in preventing and controlling of heart cerebrovascular diseases to reduce the threat of heart cerebrovascular diseases for the health of the elderly in rural areas.
