Research Background:Depression is a kind of negative emotion and mood disorder,dominantly characterized by persistent depressive mood.Patients with depression may develop some pathological changes simultaneously for a long time,including behavior and physical symptoms,which may influence their social function.Nowadays,depression already is the most important problem in human's mental health,and acts as a critical disease with the second expensive condition estimated by WHP,in 2020,just inferior to the cardiovascular disease.In China,the morbidity of depression increases up to 23.60%.Notably,the older become the big group in patients with depression,with morbidity of 6.35%?60.30%.Depression also has a fearful effect on the quality of life of the older.Sleep disorders,as an important role in somatic syndromes of depression,has been identified as "someone fails to sleep immediately,keep in deep sleep,or has high quality of sleep under the condition of sufficient time and chance",which in turn results to a bad influence on daytime function.In China,there 45.5%people are suffering from sleep disorders,especially the older(occupying about 56.7%).Actually,depression interacts with sleep disorders,which results in a vicious circle.According to recent researches,more and more older people suffer from depression and sleep disorders meanwhile.the majority of current studies focus on urban resident who live with better income and medical treatment service.However,the older living in village are more susceptible to depression and sleep disorders due to economical backward and lack of medical resources relatively.Hence,this study aims to investigate the correlation between sleep disorders and depression in the older in Yanggu village,Shandong province by Cross-sectional researchObjective:1.Inquiry on the epidemiological characteristics of sleep disorders and depression in the elderly in rural areas of Yanggu,Shandong Province.2.To further analyze the correlation between sleep disorders and depression in elderly people in rural areas of yanggu county,shandong provinceMethods:From August 2014 to December 2014,a cross-sectional research was conducted to collect general epidemiological data in 65 years and older in yanlou town,yanggu county,shandong province by face-to-face epidemiological survey.Collecting demographic data,lifestyle and health history of the target population;At the same time,the general physical examination data of the target population were collected;Fasting blood was tested and laboratory data of liver function and biochemistry were tested.The sleep disorders of the study population was assessed by the questionnaire of Pittsburgh sleep quality index(PSQI).The depression status was assessed by HAMD.Results:1.The epidemiological characteristics:the average age of 711 people is 71.17±5.144;Male,258,accounting for 36%,the average age is 71.3 ±5.1;Female,453,accounting for 64%,the average age is 71.1 ±5.2.Among the male,186(72.1%)have the education level of primary school education or below;Among the female,433(95.6%)have primary school education or below,suggesting that female have significantly lower education than men.In male,209(81.0%)were married;While the female,309(68.2%)were married,suggesting that female's married ratio is lower than men.In male,221(85.7%)is farming;While the female participants,447(98.7%)is farming,suggesting that male's farming proportion is lower than female.2.Scale results:According to the score of HAMD scale,148 people have certain depression,563 people don't have depression.In depressive patients,116 peolple is female,and 139 people had primary school education or below.It suggested that the baseline data of the population with different depression degrees were different,and the severity of depression was higher in women and those with lower education degrees(P<0.05),The PSQI score of Normal groups is 6.4,The PSQI score of may and must be depression group is 8.5.Concluded that poor sleep quality and depression have statistically significant correlation(P<0.001)(table 2).3.Without adjusting for influencing factors,logistic regression analysis showed that Sleep disorders(PSQI)can increase the risk of depression[OR:1.125,95%CI(1.077,1.174),P<0.001];Female have higher risk of depression[OR:3.431,95%CI(1.588,3.723),P<0.001];Higher education level have a higher risk of depression[OR:0.374,95%CI(0.183,0.764),P<0.001];After adjusting for age,sex,marriage and education,PSQI remained a higher risk for depression[OR:1.414,95%CI(1.066,1.163),P<0.001].Gender remained a higher risk for depression[OR:2.027,95%CI(1.286,3.194),P=0.001].4.ROC curve analysis:a logistics regression model is used for prediction.Predicted value is adopted to discern and diagnose whether one is depressed.The ROC curve is drawn as picture 2.The model AUC is 0.77,higher than 0.7,and the sensitivity is 79.14%,and the specificity is 65.65%.The model is considered to have good discernment ability.Conclusions:1 Depression and sleep disorders are prevalent among the elderly in the rural area of Yanggu Shandong province.2.sleep disorders are positively correlated with Depression disorder,and they are probable the risk factors of depression disorder among the old people in the rural area of Yanggu Shandong province.
