Analysis On The Correlation Between Sleep Quality And Blood Pressure Control Level In Elderly Hypertensive Patients

Objective:We investigate the sleep quality in hypertensive patients with 60 years old and above in Hefei,Anhui Province and Mu Qiao town,Zhengpu New District,MA' Anshan city of Anhui Province,further to analyze the blood pressure and the sleep quality in elderly hypertensive patients and the correlation about them,and to explore the influence factors of sleep quality in elderly hypertensive patients.Methods: In July 2015 and July 2016,we selected of Lotus Street Community,Qili Tong Street Community and West Park Street Community in Hefei city,and Muchang Village,Hongguang Village and Guan Tong Village in Mu Qiao town.Through the community health service centers and village clinics to obtain a list of hypertensive patients' health records,then hypertensive patients with over the age of 60 were investigated by means of questionnaire survey and blood pressure measurement.A total of 1021 cases of hypertensive patients were investigated,and the effective questionnaires were 856 cases,the effective rate was 83.8%.The self-designed questionnaire and the Pittsburgh Sleep Quality Index scales were used.It included the basic situation such as age,sex,education,health insurance and other issues and elderly hypertensive patients with sleep quality.Investigators were trained to conduct a questionnaire survey of the respondents in a one-to-one manner and to measure the blood pressure according to the standard.In accordance with the guidelines for prevention and treatment of hypertension in China 2010,the blood pressure was divided into 5 levels: normal blood pressure,high normal blood pressure,and hypertension of grade 1 level,hypertension of grade 2 levels,and hypertension of grade 3 levels.Epidata3.1 and SPSS16.0 were used for data entry and data analysis after verification.Results:(1)The average level of blood pressure in elderly hypertensive patients with4 urban was 139.98 / 80.91 mm Hg,rural was 141.25 / 80.64 mm Hg.There was no significant difference in urban and rural about the average blood pressure level(P>0.05).The level of blood pressure grading in urban elderly hypertensive patients was 38.9% and 40.8% respectively in high normal grade and hypertension of grade 1 level;The level of blood pressure grading in rural elderly hypertensive patients was 32.9% and 43.5% respectively in high normal grade and hypertension of grade 1 level;no statistical differences between urban and rural levels in blood pressure grade(?2=5.693,P=0.223).The urban of isolated systolic hypertension(ISH)patients in elderly hypertensive patients accounted for 36.1%,39.1% in rural areas,there was a statistically significant in city and rural(?2=4.788,P=0.029).(2)PSQI total score in urbanelderly hypertensive patients was 7.31±3.96,and rural areas was 9.02±3.94.There were significant differences in duration of sleep,sleep disturbance,sleep latency,sleep efficiency,subjective sleep quality and PSQI score in urban and rural areas(P<0.05).The score about duration of sleep,sleep quality,sleep efficiency,subjective sleep quality And PSQI score in the rural higher than that in the city,but sleep latency score less than that in the city.(3)There were significant differences in daytime dysfunction between elderly patients with hypertensive at different blood pressure levels(P<0.05),and no statistically significant between the other components and blood pressure levels(P>0.05).There was a significant difference in sleep efficiency between ISH patients and non-ISH patients(P<0.05).ISH patients had worse sleep than non-ISH patients.There was no significant correlation between PSQI score and its seven components in SBP and DBP of elderly hypertensive patients.However,pulse pressure was positively correlated with sleep efficiency score.(4)The single factor analysis found that gender,cultural level,occupation,medical insurance form is the influence factors of elderly hypertensive patients;the Logistic regression analysis showed that female,the new rural cooperative medical insurance,patients with hypertension over 5 years old,not taking antihypertensive drugs have poor sleep quality.Conclusions:(1)The average blood pressure level of elderly hypertensive patients in urban and rural areas was slightly lower,and the blood pressure grade was mainly concentrated at normal high level and grade 1,and the proportion of ISH patients was higher.The incidence of sleep disorders in elderly hypertensive patients is higher,and that in rural areas was higher than that in urban areas.(2)There was no significant correlation between PSQI score,seven dimensions scores and SBP,DBP in elderly hypertensive patients.(3)The elderly hypertensive patients in daytime dysfunction was increased with blood pressure levels.SBP fluctuation and pulse pressure difference affect the sleep efficiency of elderly hypertensive patients.(4)Women,low cultural level,engaged in physical labor,new rural cooperative medical insurance patients,suffering from high blood pressure?5 years of the elderly population,not taking antihypertensive drugs had poor sleep quality.
