Objective:In this study,clinical observation was conducted on the treatment of chronic glomerulonephritis with spleen-kidney qi deficiency by adding and reducing Shengyang Yiwei Decoction,so as to objectively evaluate its efficacy and safetyMetliods:The method of self-control before and after treatment was used for clinic al observation.Seventy-eight patients with chronic glomerulonephritis of spleen-kidney qi deficiency were included.Under the premise of basic treatment and symptomatic treatment,decoction of Traditional Chinese medicine was added and reduced,and Shengyang Yiwei Decoction was taken orally.The treatment process was 8 weeks,and follow-up was conducted every 4 weeks.The observation indexes were blood routine,urine routine,liver function,kidney function,24-hour urinary protein quantification(24hUP)andTCM syndrome score.Clinical efficacy was evaluated by 24hUP,urinary red blood cell(URBC)count and TCM syndrome score,and safety was evaluated by changes in liver and kidney function before and after treatment.The data were statistically analyzed by SPSS21.0Results:(1)Proteinuria:The assessment indicator was 24-hour urinary protein quantification.There were statistically significant differences before and after 24hUP treatment with different courses of treatment(4 weeks and 8 weeks)(P<0.05).There were statistically significant differences in 24hUP after 8 weeks of treatment with different degrees of proteinuria(divided into H,M and L groups according to the degree of proteinuria)compared with that before treatment(P<0.05).Different TCM syndromes(divided into tw,o groups with and without standard syndromes)showed statistical difference in 24hUP after 8 weeks of treatment compared with that before treatment(P<0.05).(2)Hematuria:After 8 weeks of treatment,URBC count was P<0.05 compared with that before treatment,and the difference was statistically significant.(3)Western medicine curative effect:after 8 weeks of treatment,7 cases were clinically controlled,49 cases were significantly effective,15 cases were effective and 7 cases were ineffective,with a total effective rate of 91.03%.(4)TCM curative effect:The changes of TCM syndrome score before and after treatment were statistically significant(P<0.05).After 8 weeks of treatment,TCM curative effect:9 cases were cured clinically,18 cases were obvious,45 cases were effective,and 6 cases were ineffective,with the total effective rate of 92.31%.(5)Safety evaluation:No adverse reactions were observed throughout the treatment.After 8 weeks of treatment,blood routine(Hb,PLT)and liver function(ALT,AST)and eGFR were not abnormalConclusion:(1)Adding or reducing Shengyang Yiwei Decoction in the treatment of chronic glomerulonephritis of spleen-kidney qi deficiency can significantly reduce the 24-hour urinary protein quantity of patients,and has a significant therapeutic effect on urinary red blood cells.(2)Adding or reducing Shengyang Yiwei Decoction can significantly improve the clinical symptoms and signs of chronic glomerulonephrit's with spleen-kidney qi deficiency.(3)Adding or reducing Shengyang Yiwei Decoction has no obvious effect on eGFR,liver function(ALT,AST)and blood routine(Hb,PLT).It,s safe and reliable.
