Research Purpose In order to improve the level of clinical specialist service of county general hospital.This research builds a set of clinical specialist service capacity that applicable to general hospitals in County based on the Donabedian evaluation framework and a combination of subjective and objective evaluation methods.Evaluation indicator system,and the indicator system is used to evaluate the clinical specialties of the "W hospital" of the tertiary-grade A class general hospitals in county of the northeast of Guangzhou,and provide reference opinions for the improvement of the clinical specialist service capacity of the hospital.Research Method Used the literature research method,interview method and Delphi Method to design the framework of indicator system,and the Analytic Hierarchy Process was used to determine the index weight coefficient.Based on the evaluation indicator system,the service capacity of clinical specialties of the research hospital were evaluated by weighted TOPSIS and RSR.Research Results (1)The Delphi Method and the Analytic Hierarchy Process constructed an indicator system that consisting of three levels,including 3 class first indicators,8 class second indicators,and 28 class third indicators.The weighting of the "structural service capacity" indicator of the class first indicator is 0.1373,the weighting of the"process service capacity" indicator is 0.6232,and the weighting of the "resulting service capacity" indicator is 0.2395.Among the class second indicators,the most important indicator is "service quality",followed by it is the "technical level" and again the "service efficiency".(2)The application of the evaluation indicator system,the overall service capacity of hospitals in research hospitals has shown an upward trend year by year,of which the largest increase in 2017;from the perspective of each clinical specialists,the ranking of each specialty is relatively stable.Obstetrics and gynecology had the strongest service ability,followed by ophthalmology,cardiovascular medicine,pediatrics,neonatology and general surgery,and the three last are oncology and hematology,neurosurgery and ICU.There is an imbalance in the level of development of clinical specialist services in research hospital,especially between obstetrics and gynecology and other specialties,followed by problems such as the need to strengthen talent construction and low technical service levels.(3)According to the evaluation results,put forward countermeasures to improve the capacity of clinical specialist services in "W hospital" in county general hospital.First,improve the efficiency of specialist management and provide patients with more satisfactory specialist services.Second,improve the level of specialist services,and form a core competition between technology and quality.Third,strengthen the support of weak specialists and achieve the balanced development of specialist service capacity.Finally,establish a talent incentive mechanism and optimize the overall structure of the talented technical team.Research Conclusions (1)The evaluation indicator system has relatively reasonable in structure,wide in coverage and complete in content.(2)The results of the evaluation indicator system determined by the Delphi Method are reliable.(3)Based on expert judgment,the Analytic Hierarchy Process is adopted to determine the indicator weights scientifically and reasonably.(4)The evaluation indicator system is applied to the research hospital,and the evaluation results basically conform to the actual situation of the hospital,indicating that the indicator system can be used to evaluate the clinical specialty service capacity of general hospitals in county.
