Development Of A Service Capability Evaluation Index System For County General Hospitals

[Objective] To establish an index system that can evaluate county general hospitals' service capability comprehensively,rationally,scientifically and fairly based on China's policy documents on medical reform and the implementation effect of the hospital level appraisal system in accordance with the related research achievements of domestic hospital evaluation,then make a detailed analysis of Chinese county public hospitals' actual service capability using the index system based on the data of representative county general publics,and adjust the weights of indexes in the index system to evaluate and understand the gap in service capability between different levels of county general hospitals as well as the real level of their service capability,and finally summarize the operating conditions of this index system.[Method] In this study,we screened the indexes related to evaluation of hospital service capability summed up in the part of literature review by group discussion and expert consultation,preliminarily building an evaluation system for hospital service capability;distributed questionnaires to experts by Delphi method for consultation about determination of indexes and weights for the service capability evaluation index system.Then we collected the questionnaires and analyzed the data,to determine indexes and weights for the service capability evaluation index system.We selected hospitals and distributed questionnaires to them for a survey of their service capability,and then extracted information from the questionnaires after collection from the hospitals.In accordance with the service capability index system,we processed the target hospital information by indexation,figuring out the weight value of various hospital indexes,analyzing the actual development status of county hospitals' service capability.[Results](1)A service capability evaluation system consisting of 9 primary indexes and 36 secondary indexes was established for hospitals at the same level,which could be used to comprehensively measure the service capability of the county general hospitals at the same level;(2)data was collected from four county hospitals in four provinces including two county secondary hospitals and two county tertiary hospitals;(3)the hospital data was analyzed for an understanding of the development tend of the service capability of the hospitals at the same level;(4)the index system was upgraded to an evaluation system consisting of 7 primary indexes and 33 secondary indexes that could be used to evaluate the service capability of the county general hospitals at different levels;(5)it was found after hospital data alignment that there was a difference in service capability between different levels of hospitals;(6)the development status of hospital service capability was summarized,with suggestions put forward for an improvement of the service capability.[Conclusions](1)County secondary and tertiary general hospitals' service capability has been improved;(2)there is still a big difference in service capability between county secondary and tertiary general hospitals;(3)the evaluation index system is scientific and effective;(4)the index screening method is rational,so is the weight determination method;(5)the evaluation index system is easy to operate;(6)the index system needs further improving due to the impact of individual factors and the sample size.
