Objective:To introduce a new nucleus fragmentation method applied in small incision cataract surgery in the hard nucleus cataract,which compare with phacoemulsification,analyze it's advantages and disadvantages,so as to provide a choice for clinical decision-making of hard nucleus cataract surgery.Methods:June 2019 to December 2019,a total of 56 patients(60 eyes)department of the Second Affiliated Hospital of Kunming Medical University according to Emery's nuclear hardness classification method,Patients were randomly divided into two groups:group A 29 patients(30eyes)were treated with phacoemulsification,group B 27 patients(30eyes)were treated with small incision cataract surgery.The uncorrected distant visual acuity(UDVA),best corrected distant visual acuity(BCDVA)at one day,one week,one month,and three months after operation;surgically induced astigmatism at one day,one week,one month,and three months after operation;the number of central corneal endothelial cells and at one month,three months after operation;operation time,intraoperative and postoperative complications.The data obtained are statistically soft SPAA 25.0.Results:There was no statistically significant difference between the two groups(P>0.05)in preoperative visual acuity(UDVA?BCDVA),the visual acuity(UDVA?BCDVA)was no statistically significant difference between the two groups(P>0.05)which at one day,one week,one month,and three months after operation,the visual acuity(UDVA?BCDVA)of two groups in different periods preoperative and postoperative were compared,the difference was statistically significant(P<0.05)There was no statistically significant difference in various periods postoperative surgically induced astigmatism between two groups(P>0.05).There was no statistically significant difference in corneal endothelial cell density between the two groups in preoperative(P>0.05);the corneal endothelial cell density of group B was higher than that of group A at one month and three months after operation(P<0.05).the corneal endothelial cell density of two groups in various periods before and after operation was compared,and the corneal endothelial cell density after operation was lower than that before operation,the difference was statistically significant(P<0.05),the loss rate of corneal endothelial cells in group B was lower than group A at one month,and three months after operation,the difference was statistically significant(P<0.05).Intraoperative and postoperative complications and operative time,posterior capsule rupture was found 2 eyes(6.67%)in group A,posterior capsule rupture was not found in group B;On the first day after operation,7 eyes(23%)had corneal edema of grade 3 or above,3 eyes(10%)had corneal edema of grade 3 or above,finally,the cornea is transparent;2eyes(6.67%)had iris injury in group A,5 eyes(16.67%)had iris injury in group B;the average operation time was 7.83 min in group A,the average operation time was 4.65 min in group B.Conclusion:1.The side incision assisted nucleus-chopping technique is applied in the small ncision cataract surgery,which can achieve the similar postoperative effect as phacoemulsification.2.small incision cataract surgery has the advantages of shorter operation time,better protection of corneal endothelium and posterior capsule of lens,lower operation cost,lower requirements for surgical equipment,small incision cataract surgery has more advantages in the treatment the hard nucleus cataract and large-scale centralized anti-blindness operation.
