The clinical research of coaxial small incision cataractphacoemulsification and intraocular lens implantationThe clinical research of coaxial small incision cataract phacoemulsification and intraocular lens implantationWith the development of science and technology, cataract surgery brought good news for cataract patients due to the constant improvement of advanced equipment and operation skill. Those techniques Such as ultrasonic emulsifying equipment improvement, surgical incision shrinks, the emergence of new artificial crystal make cataract surgery become better and approach perfection day by day. It becomes one development trend of cataract surgery that how to lessen cataract surgery incision in order to eliminate astigmatism to the utmost and improve the safety of the operation.Cataract supersonic emulsification surgery have improved either in surgical methods, incision choosing or in ultrasonic emulsifying equipment. Now the requirement of the development of the surgery is more higher, and have preliminary achieved shift to dioptrical surgery. It needs much better visual quality, much smaller incision eliminating the astigmatism effectively caused by operation, less energy release, less damage, and faster recovery. High quality of visual enjoyment is the goal we are striving toward. Objective:prospective study. We choose 80 cases of age-related cataract encountering in ophthalmologic department in the second hospital of Jilin University. All the cases, excluding other diseases, was randomly devided into two groups. The first group was operated by 2.2mm small incision cata-ract phacoemulsification and intraocular lens implantation, and the second group used traditional 3.2mm incision coaxial cataract phaco emulsification and intraocular lens implantation. All the cases were operated by only one Optician with Infiniti superultrasonic equipment made by American Alcon company. All the intraocular lens used were aspheric surface AcrySof intraocular lens. The following index were observed:superultrasonic accumulating energy,This study intends to by observing 2.2 mm microincisional cataract coaxial phacoemulsification and intraocular lens implantation, and with the clinical effect of traditional 3.2mm incision coaxial phacoemulsification comparison for guidance, surgical clinical provide objective material.MethordThis study was a prospective observational series of 80 eyes of 50 patients with age-related cataracts in the second hospital of Jilin university. There were two groups each with 40 eyes. GroupⅠeyes were treated with microcoaxial phacoemulsification using a 2.2 mm temporal clear corneal incision; groupⅡeyes were treated with phacoemulsification using a 3.2 mm temporal clear corneal incision. All the patients were using American Alcon company produces the Infiniti ultrasound emulsifying equipment machine, and All the patients were surgeried by the same performer and All the patients were implanted AcrySof artificial crystal aspherical SN60WF (AcrySofIQ)。Recording cumulative dissipated energy and recording postoperative recovery observed the bare eye sight, astigmatism change and complications at day 1, week 1and month 1 postoperatively, checking contrast sensitivity at month 1 postoperative.ResultThere was significant difference in the bare eye sight of two groups at the time of Postoperative 1 day (p=0.047, p<0.05),and groupⅠis better than groupⅡ, That means incision groups of patients recover faster; There was significant difference in the surgically induced astigmatism of two groups at the time of Postoperative 1 day,1 week and 1 month; and group I is lower than groupⅡ; There was no significant difference in other aspectsConclusionsIn comparison to the eyes which were treated with phacoemulsification using a 3.2 mm temporal clear corneal incision, the eyes which were treated with phacoemulsification using a 2.2 mm temporal clear corneal incision have less astigmatism, visual improved quickly. There were no no difference in cumulative dissipated energy and contrast sensitivity. Microincision cataract surgery can be realized by intraocular Lens of AcrySofIQ and it is safe and feasible.
