Objective:Understand the basic information,ART work condition,and prospects of anti-AIDS medical workers at the provincial,municipal,county,and township which 4 different levels and places of detention in Yunnan province,explore the factors that affect the work performance of anti-AIDS prevention,and propose strategies and ways to strengthen the capacity of medical personnel.To conclude which comprehensive model is beneficial to the epidemic development in Yunnan province.Methods:In this study,a total of 361 samples were selected according to the method of selecting one medical staff for every 500 patients treated to participate in the questionnaire survey.Through descriptive analysis and logistic regression analysis of the questionnaire results,to understand the basic situation,job satisfaction,and factors influencing the effectiveness of anti-AIDS medical workers.Results:A total of 361 people participated in the questionnaire survey of designated medical institutions medical staff for HIV/AIDS treatment in Yunnan province,including 22 people from the provincial level,92 people from the city level,144 people from the county level,60 people from the township level,and 43 people from custody department.They undertake tasks such as medical diagnosis and referral,psychological counseling for AIDS-infected patients and patients;behavioral intervention for high-risk groups and health consultation,etc.1.Basic information:The ratio of males to females in anti-AIDS is 1:2,the average age is 38.9±9.4 years,and the age range is mainly 40-50 years(37.4%);the Han nationality accounts for 65.7%;the undergraduate degree and above account for 50.4%,among them,master's degree accounted for 2.8%;clinical medicine majors were 183(50.7%)people;the distribution of professional titles was mainly junior(34.9%),and the titles of deputy senior and above accounted for 15.2%.2.Work status:209 are full-time staff,accounting for 57.8%;There are 246(68.1%)medical personnel in the establishment;229(63.4%)people who believe that the number of medical staff they are treating is insufficient,and those with less than 5 years of AIDS prevention work experience are 169 people,accounting for 46.8%of the total;medical staff working more than 8 hours a day accounted for 35.7%;128(35.5%)people who often worked overtime in their daily work;There are 127(35.2%)anti-AIDS medical staff reported to the leadership that the work cycle was about one month on average;49.9%of whom work affects family life to a certain extent;85%of the respondents believe that "patients are difficult to manage" has become the most important obstacle to the work.3.Treatment situation:The actual average income of the people under investigation for HIV prevention was 6021 yuan,and the expected average income was 9638 yuan,with a ratio of 1:1.6;36.5 percent are earning more than they did last year,There are 83(22.9%)people whom income level was the lowest among employees with the same qualifications in the unit.4.Job satisfaction:For those who are willing to continue to work in ART,275(76.1%)choose "willing";according to the sense of accomplishment index 1-10 points(low-high),more than 8 points have 194 people,account for 35.7%.5.Training situation:A total of 341 of the 361 respondents have participated in antiviral therapy-related training,accounting for 94.5%of the total,and 35(9.7%)of them have attended more than 20 times of training;There are 126(34.9%)people who feel that they have insufficient knowledge of ART-related knowledge;the most frequently paid attention to the content of training is antiviral clinical treatment(91.4%),ART management(81.1%),enrollment therapy and care skills(80.8%),etc.6.Analysis of influencing factors of work effectiveness:The difference of different supervision and inspection cycles in work effectiveness is statistically significant(X2=8.631,P=0.003).The logistic regression model results show that the shorter the supervision work cycle of superiors to treatment sites(less than 1 month),the higher productive of the work(OR=1.947,95%CI:1.245-3.044).7.Detention department:The medical personnel have a high degree of education and professional counterparts,but 74.4%of ART staff are part-time,and 58.1%have less than 5 years of work experience.There is still much room for improvement in their own knowledge and skills.It is suggested that attention should be paid to strengthening the construction of the full-time aids-prevention team of the custody department in this province,strengthen the training of medical staff,gradually improve the diagnosis of medical staff.Conclusion:1.Current situation:The overall configuration of the medical staff engaged in AIDS prevention in Yunnan province tends to be reasonable but needs to be improved;the work pressure and burden of the medical staff are high;the salary level and welfare benefits are low;The satisfaction and expectation of AIDS prevention staff to ART work should be improved.The form and content of the training should be further enriched and improved.2.Work effectiveness analysis:Analysis of statistical results,it is recommended that leaders strengthen the importance of preventing HIV,regularly supervise and urge medical staff to improve work efficiency;Team leader increase the frequency of department meetings as much as possible,and help medical staff solve difficulties in work in a timely manner,improve the professional ability and comprehensive quality of AIDS prevention personnel and carry out their work in an orderly manner.3.Countermeasures and suggestions:optimize the allocation of human resources,integrate the working mechanism;improve the welfare mechanism,promote the sinking of the center and establish a stable workforce;higher-level departments pay more attention to the use of incentive mechanisms,focus on job burnout;pay attention to professional knowledge and skills training "Quality" and "Quantity" effectively improve personnel management capabilities;Improve the cooperation between detention places and local institutions,and explore the establishment of a long-term working mechanism.
