Background:At present,there is still a lack of general practitioners in our country,the foundation is still weak,the multi-level needs of patients cannot be met,and the professional value of general practitioners is not fully reflected.People don't know enough about general practitioners,the concept of medical treatment has not changed,there is a crisis of trust between general practitioners and patients,and the structural mismatch of general medical human resources supply and medical service demand has caused many contradictions.How to let the limited general medical resources meet the needs of patients to the greatest extent,and at the same time to enhance the enthusiasm of general practitioners;to improve the dislocation of supply and demand,to achieve supply and demand matching is its key breakthrough pointObjective:Based on the knowledge of general practitioners and patients,the KANO model was used to investigate the demand attributes of general practice medical services in Guangdong province,and the demand attributes were classified.On this basis,the author puts forward some suggestions to perfect and improve the general medical service,optimize the allocation of general medical resources,and improve the efficiency and service quality of general medical serviceMethod:Using literature analysis method,we collect relevant policy documents and domestic and foreign literature on general practice at the national and local government level,understand the research progress of different scholars at home and abroad,and provide sufficient theoretical basis for the research of the subject.general practice service demand attributes based on the cognition of general practitioners and patients are understood through a questionnaire survey based on KANO model.Using the interview method to understand the development of general medical services,existing bottlenecks and development suggestions,and then the text analysis of the content of supplementary improvement.On the basis of KANO model analysis,through two-dimensional service demand attribute classification,Better-Worse coefficient calculation and two-dimensional matrix analysis,this paper analyzes the demand of general medical service between supply and demand from qualitative and quantitative level,finds out the difference and matching point between supply and demand,and determines the order of priority of general medical service demand.The Epidata3.1 was used to input the data,and the SPSS22.0 was used to analyze the general situation of general medical service demand between supply and demand,the difference of different demographic factors and the demand of general medical service between different regions.Statistical methods include:descriptive analysis,reliability and validity test,chi-square test and Fisher test,with P<0.05 as the difference is statistically significant.Results:1.The classification results of the attributes of general medical service demand between supply and demand in Guangdong province:general practitioners have 4 charm requirement attributes,11 expectation requirement attributes,9 essential requirement attributes and 1 non-differential requirement attribute;patients have 7 charm requirement attributes,13 expectation requirement attributes,4 essential requirement attributes and 1 non-differential requirement attribute.2.The difference between supply and demand of general-practice medical service under multi-dimension2.1 Differences in demand attributes between supply and demand of general medical services between different demographic factors.Different gender,age,household registration,title,monthly income,unit based on general practitioner cognition of general practice medical service demand attributes are different;different gender,age,education,household registration,monthly income,payment methods,occupation of patients with general practice medical service needs are different.2.2 Cognitive differences between supply and demand in different regionsFrom the point of view of service demand items,there are 17 items at the general practitioner level and 18 items at the patient level;from the point of view of both supply and demand,there are three significant indicators of the difference between supply and demand in each region of Guangdong Province;from the regional point of view,in general,the Pearl River Delta and eastern Guangdong are more inclined to charm and expectation demand attributes,and the northern Guangdong and western Guangdong regions are more inclined to essential demand attributes.2.3 There were cognitive differences between supply and demand for general medical services.The results of chi-square test of two independent samples showed that there were statistically significant differences among 16 general medical services.3.The most important Better-Worse factor the demand attribute of both supply and demandThe key factors affecting the rapid increase in general practitioners' satisfaction and the significant decrease in their satisfaction are,in turn,the issuance of a health prevention manual(0.71)and the establishment of a competitive mechanism to promote the quality of general practitioners' services(-0.67).The key factors to improve the satisfaction of patients' general medical service demand and reduce their dissatisfaction quickly are:providing general medical knowledge base(0.73)and providing Internet service tools,accurate matching specialist(-0.69);4.Two-dimensional matrix analysis and priority ranking of general medical service demand between supply and demandThe essential demand attribute service index is the priority satisfied area,the expected demand attribute service index is the key optimization area,the charm demand attribute service item is the area to be improved,the non-differential demand attribute is the secondary improvement area.Essential demand attribute is the most important attribute of both supply and demand,then the reverse demand attribute is removed,then the expected demand attribute is satisfied,then the charm demand attribute is satisfied,and finally the non-differential demand attribute is M>R>O>A>I5.Supply and Demand Matching Results of General Service ServicesBased on the KANO model,there are 12 matching points of supply and demand of general medical service in Guangdong Province,and the supply and demand parties agree on the index of "full cycle,diversified health management service,Internet medical and health service,individualized and differentiated general medical service ".Conclusion:The KANO model energy analysis of guangdong province general practitioners and patients supply and demand general medical service demand attribute types,according to the Better-Worse coefficient level to find the supply and demand both general medical service demand satisfaction index elements,according to the supply and demand matching results and demand hierarchy conceptual framework,achieve general medical service demand priority ranking,based on this analysis and find supply and demand matching points and dislocation points.A reasonable and effective matching of supply and demand of general medical services can meet the needs of patients with different needs,and maximize the value of general practitioners' medical services,which is conducive to gradually reducing medical costs,improving service efficiency and satisfaction between doctors and patients,and enhancing the competitiveness of general medical service industry.The conceptual framework of the demand level of general medical service between supply and demand is preliminarily constructed,which should guarantee the necessary attributes of general medical service demand,perfect the expected attributes and enhance the charm attributes.The demand attribute of general medical service is spiral rise,which requires continuous dynamic periodic monitoring and promotion.
