In the process of continuous development of the Yangtze River Delta integration, the unbalanced development of people's livelihood public services such as basic medical and health services has affected the flow of various elements in the region and restricted the development of higher quality of regional integration. How to break through the bottleneck of development and narrow the regional gap is a realistic problem that needs to be solved urgently in the process of promoting the balanced development of basic medical and health services and achieving higher quality integration in the Yangtze River Delta. This study takes 26 cities in the Yangtze River Delta as the research object, collects relevant data on the basis of literature review and constructs a reasonable index system, and uses the entropy method and coefficient of variation to evaluate the basic medical and health service levels of 26 cities in 2008-2017.Based on the exploratory spatial data analysis, the spatial measurement model is introduced to empirically analyze the influencing factors of regional differences in basic medical and health service levels in the Yangtze River Delta. The main conclusions are as follows:First, there are obvious differences within the region of the basic medical and health service level in the Yangtze River Delta, but the gap between cities is significantly reduced.Second, the basic medical and health service level in the Yangtze River Delta has a positive correlation in the global scope, and the correlation shows a volatility upward trend, showing a clear agglomeration pattern in space.Third, local government financial pressures and local government competition have a significant inhibitory effect on the improvement of basic medical and health services.Government spending preferences, fiscal budget revenue, and population density have significantly promoted the improvement of basic medical and health services. The impact of development levels and urbanization levels on basic health care services is not significant.Fourth, the spatial spillover effect of basic medical and health service levels in the Yangtze River Delta region is negative. In addition, local government competition shows significant negative spatial externalities, while urbanization levels show significant positive spatial externalities.Based on the above research conclusions, this paper puts forward the following suggestions:First, improve the public finance system and standardize the local government fiscal expenditure;second, optimize the government evaluation mechanism to guide the local government's healthy competition;third, optimize and adjust the resource allocation, deepen The reform of the medical and health system;fourth,to play a positive spatial spillover effect and improve the supervision and evaluation system.
