Clinical Study Of Yigong Yangbao Decoction In The Treatment Of Delayed Menorrhea With Premature Ovarian Insufficiency Due To Kidney Deficiency

Objective:To observe the efficacy and safety of Yigong Yangbao Decoction in the treatment of premature ovarian insufficiency with kidney deficiency,and to discuss its clinical efficacy,so as to provide more effective measures for the treatment of this disease.Methods:From February 2019 to December 2019,90 patients who were admitted to the gynecological outpatient department of Ruikang hospital affiliated to Guangxi university of traditional Chinese medicine and met the inclusion criteria were randomly divided into treatment group A,treatment group B and control group?30 cases each?.The patients were followed up for three months after treatment with Yigong Yangbao Decoction combined with Climen,Yigong Yangbao Decoction and Climen for three months,to observe and record the changes of TCM symptom scores,serum FSH,LH,E₂and AMH levels,comprehensive curative effect of diseases,TCM syndrome efficacy,menstrual cycle recovery and medication safety.SPSS22.0 statistical software was used to analyze the data and make a conclusion based on the statistical results.Results:In the course of the study,a total of 4 cases fell off due to various factors,and the actual number of cases was finally completed,including 28cases in the treatment group A,29 cases in the treatment group B and 29 cases in the control group.?1?Comparison of changes in basal FSH,LH and E₂values:after treatment,the values of basal FSH and LH in the three groups decreased significantly compared with before treatment,while E₂increased significantly compared with before treatment.Treatment group A had the best effect on improving patients'FSH level,LH level and E₂level,the control group had the second best effect on improving patients'FSH level,and the treatment group B had the worst effect on improving patients'FSH level.Treatment group B and control group had similar efficacy in improving LH and E₂levels.?2?Comparison of AMH level changes:there was no statistical difference in the AMH values before and after treatment among the three groups.AMH levels in treatment group A,treatment group B and control group did not improve significantly in A short period of time.?3?Comparison of scores of major menstrual disorders in late menstrual period:three treatment methods can improve the symptoms of menstrual disorders in late menstrual period.The treatment group A had the best effect in improving the menstrual disorders such as late menstruation of kidney-deficiency POI,followed by the treatment group B,but there was no significant difference between the treatment group A and the treatment group B.The control group had the worst efficacy in improving the late menstrual period of kidney asthenia POI and other menstrual disorders,and the efficacy of treatment group A and treatment group B was significantly different from that of the control group.?4?Comparison of scores of single syndromes of secondary diseases in traditional Chinese medicine:the three treatment methods can improve soreness and weakness of waist and knees,dizziness,tinnitus,dim complexion,libido loss and more frequent nocturia in POI patients with kidney deficiency syndrome.The curative effect of treatment group A and treatment group B in the improvement of soreness and weakness of waist and knees,dizziness,tinnitus and more frequent nocturia in patients with POI syndrome of kidney deficiency was similar,and both of them were better than the control group.?5?Efficacy comparison of TCM syndromes:the total effective rate of treatment group A,treatment group B and control group was96.42%,89.66%and 75.86%,respectively.Treatment group A had the best efficacy of TCM syndromes in improving kidney-deficiency POI,followed by treatment group B,and the control group had the worst efficacy in improving kidney-deficiency POI.?6?Comprehensive efficacy comparison:the total effective rate of comprehensive curative effect of diseases in treatment group A,treatment group B and control group was 89.29%,86.21%and 68.97%,respectively.The clinical efficacy of POI in treatment group A and treatment group B was similar,and the comprehensive curative effect of treatment group B was significantly better than that in control group.?7?Drug safety analysis:no obvious adverse reactions were found in the three groups during and after the treatment,and no obvious abnormalities were found in the results of the three major routine examinations,liver and kidney functions,electrocardiogram and other safety indicators.Conclusion:Yigong Yangbao Decoction can adjust the menstrual cycle of patients with kidney-deficient POI,improve the menstrual amount,lumbar and knee tenderness,dizziness,tinnitus,loss of libido and other clinical symptoms,and greatly improve the quality of life of patients.It can also regulate hormone levels.Although it cannot increase AMH in a short time,it can effectively reduce serum basal FSH and LH levels and increase serum basal E₂levels.Compared with Climen,its efficacy in improving LH and E₂levels is similar to that of Climen,but it is obviously superior to Climen in improving TCM syndromes and comprehensive efficacy,and it has no side effects and is safe,which is worthy of clinical application and promotion.
