Objective To explore the effects of nursing intervention based on empowerment theory on the self-management ability,social and psychological adaptation level,skin around ostomy,and ostomy complications of preventive enterostomy patients.So as to provide a theoretical basis for improving self-management and psychosocial adaptability of patients with preventive enterostomy.Methods A random number table method was used to select 108 patients with qualified colorectal prophylactic stoma in the first abdominal department and the second abdominal department in a tertiary oncology specialist hospital in Hunan Province from April to September 2019,54 cases in each group.The control group was given routine nursing measures,and the intervention group was given a 12-week empowerment intervention based on the routine nursing measures.The two groups were collected three times on the day before surgery(T0),one month after discharge(T1),and three months after discharge(T2).The general data questionnaire,intestinal stoma self-management ability questionnaire,stoma patients' psychosocial adaptation scale,and skin score around the stoma were used to analyze the effects of the empowerment intervention.Using SPSS20.0 statistical software to analyze the data obtained,P <0.05 was statistically significant.Results 1.Before the intervention(T0),there was no significant difference in baseline,general information,stoma-related data,stoma self-management ability,and socio-psychological adjustment level of the two groups(P> 0.05).2.After the intervention(1)Comparison of self-management capabilities:There were no significant differences in the scores of the two groups at T0 time point and the psychological management dimension at T1 time point(P> 0.05).The differences between the total scores of the two groups at T1 and T2 time points and the scores of the other dimensions were statistically significant(P <0.05)).There was an interaction between the time main effect and the intervention main effect of the total self-management ability scores and scores of each dimension in the two groups of patients(P <0.05).(2)Comparison of socio-psychological adaptation of stoma:There were no significant differences in the scores of the two dimensions of the two groups at time T0,and the differences in the dimensions of social life adaptation at time T1 and T2(P> 0.05).There was an interaction between the time main effect and intervention main effect of the total sociopsychological adaptation score and scores of each dimension in the two groups of patients(P <0.05).(3)Comparison of ostomy-related nursing knowledge:There was no statistically significant difference in the total scores of ostomy-related nursing knowledge and the dimensions of the two groups at time T0,and the scores of diet-related knowledge dimensions at the time of T1(P> 0.05).There were statistical differences in the total scores and the scores of each dimension of the two groups at T1 and T2.Significance(P <0.05).(4)Comparison of the number of stoma complications and skin scores around the stoma:There were statistically significant differences in the number of complications and skin scores around the stoma between the T1 and T2 groups(P <0.05).There was an interaction between the time main effect and intervention main effect of the skin score around the stoma between the two groups of patients(P< 0.05).(5)Comparison between two groups of stoma replacement methods at different time points:There was no significant difference in the stoma replacement method between the two groups of patients at T1 time point(P> 0.05),and there was a significant difference in the stoma replacement method between the two groups of patients at T2 time points(P <0.05).Conclusion The application of nursing intervention based on the empowerment theory can improve the self-management ability of the patients with preventive enterostomy to a certain extent,the level of psychosocial adaptation,and the knowledge of nursing related to ostomy,and can reduce the incidence of ostomy complications and improve the surrounding ostomy Skin condition.
