| Objective:Discussing the clinical effect of Chaihu plus keel Oyster Decoction in treating depression with stagnation of liver qi,providing theoretical basis for clinical treatment of depression,and expanding the idea and theoretical method of treating depression with stagnation of liver qi.Method:Depressive patients with stagnation of liver qi were randomly divided into control group and treatment group,41 cases in control group were treated with escitalopram oxalate and 42 cases in treatment group were treated with Chaihu plus keel Oyster Decoction on the basis of control group.The treatment period of both groups was 8 weeks.The evaluation indexes of Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA)and TCM syndrome score,were evaluated at baseline and 1,2,4 and 8 weeks after the treatment,respectively,the determination of serum BDNF level is evaluated at baseline and 8 week,to observe the efficacy of Chaihu plus keel Oyster Decoction in the treatment of mild and moderate depression,and extract its treatment target of depression with liver qi stagnation,the characteristic symptoms of depression were analyzed.Results:1.Clinical efficacy:after 8 weeks of treatment,there was a significant difference between the treatnent group and the control group(92.86%V 80.48%,P=0.004).? TCM syndrome effect:after 8 weeks of treatment,there was significant difference between the treatment group and the control group(95.24%v78.05%,P=0.001)2.CDHAMD scale score:the HAMD scale score in the treatment group and the control group decreased significantly(P<0.0001,P=0.004)compared with that before treatment in the first week,and the trend continued to the eighth week;at each observation point,there was significant difference between the treatment group and the control group(P<0.01).?HAMA scale score:the HAMA score of the treatment group decreased significantly from the first week of treatment(P<0.001),while that of the control group decreased from the second week of treatment(P<0.001);at the fourth and eighth weeks of treatment,there was statistical difference between the groups(P<0.05).?TCM syndrome score:the TCM syndromes score of treatment group decreased significantly from the first week of treatment(P<0.001),while that of the control group decreased from the second week of treatment(P<0.001);the control group had significant difference between 2 weeks,4 weeks and 8 weeks after treatment(P<0.01),the treatment group had significant difference between 1 week,2 weeks,4 weeks and 8 weeks after treatment(P<0.01).?Serum BDNF level:the serum BDNF level of the two groups increased significantly(P<0.0001)at the end of the 8th week compared with baseline;there was significant difference between the groups at the end of the 8th week(P<0.01).3.The scores of HAMD,anxiety factors,sleep factors and despair factors decreased with time,and interacted with medication with time(P<0.05)4.In the treatment group,insomnia was the best factor to improve TCM syndromes,followed by dry mouth,sweating and muscle pain.5.During the study,there were no serious adverse reactions in the two groups.There was a significant difference between the treatment group and the control group(11.90%v 39.02%,P=0.004),and no special western medicine was given.The symptoms disappeared after 8 weeks of continuous observation.Conclusion:1.Chaihu plus keel Oyster Decoction combined with escitalopram oxalate and escitalopram oxalate alone can improve depression rapidly,and the total clinical effect of Chaihu plus keel Oyster Decoction combined with escitalopram oxalate is better than escitalopram oxalate alone.2.The effective improvement of depression symptoms by Buplenrum plus keel Oyster decoction may be related to the improvement of target symptoms such as anxiety,sleep and despair state in depressed patients.3.Chaihu plus keel Oyster Decoction combined with escitalopram oxalate in the treatment of depression compared with the single use of escitalopram oxalate adverse reactions occurred less.. |
