Objective:To explore the clinical efficacy of the treatment of obesity-type non-alcoholic fatty liver disease by Fuer Bigu(intermittent fasting)method based on Wenyang Yiqi Formula,to clarify its health effects,to provide a clinical basis for this treatment and a new idea of this method. Methods:This study reviewed the fatty liver classification, fat parameters, blood lipid index, body shape index, lipid index, liver function index,TCM syndrome quantitative score and safety index of patients diagnosed as non-alcoholic fatty liver disease with obesity in the outpatient department of the Obesity Specialty and Liver Research Institute of Hubei Hospital of TCM from June 2019 to January 2020.SPSS21.0 software was used for statistical processing to analyze and compare the clinical efficacy of patients after 12 weeks of intermittent fasting intervention.Results:(1)According to the cumulative time for patients to complete light fasting within 12 weeks, and the criteria of the inclusion and exclusion of this study, the 90 patients included in the analysis were divided into three groups, namely group A: Complete 18 to 24 days of light fasting,a total of 30 cases;Group B: 30 cases of light fasting completed 12-17 days;Group C: 30 cases of light fasting completed 6-11 days.(2)After 12 weeks of treatment, the three groups were compared: Group A: 8 cases of marked effect,19 cases of effective,3 cases of ineffective, total effective rate 90.0%;Group B: 3 cases of marked effect,18 effective,9 cases of ineffective, total effective rate 70.0%;Group C: 0 cases markedly effective,10 cases effective,20 cases ineffective, the total effective rate was 33.3%.The results of the three groups were compared by rank sum test and compared with each other,p<0.05,the difference was statistically significant, indicating that the treatment effect of group A was better than group B,and the treatment effect of group B was better than group C.(3)After 12 weeks of treatment, compared with the degree of fatty liver, group A was severe in 2 cases, moderate in 4 cases, mild in 10 cases, and no 14 cases.Compared with before treatment, the degree of improvement was obvious, the difference was statistically significant(p<0.05)In group B,there were 3 severe cases,7 moderate cases,11 mild cases, and no 9 cases. The degree was improved compared with before treatment, and the difference was not statistically significant(p>0.05);5 cases in group C were severe, moderate 11 Among them,11 cases were mild, and no 3 cases, the degree was improved compared with before treatment, the difference was not statistically significant(p>0.05).Comparison between groups: the difference between group A and group C was statistically significant(p<0.05),the difference between group A and group B,group B and group C was not statistically significant(p>0.05).(4)After 12 weeks of treatment, the total scores of TCM syndromes of the three groups were significantly reduced compared with before treatment, and the difference was statistically significant(p<0.05);the comparison between groups: the efficacy of group A was better than that of group C,the difference was statistically significant(p<0.05);the difference between group A and group B,group B and group C was not statistically significant(p>0.05).Compared with before and after treatment in the same group: quantitative scores of patients in group A were significantly reduced, such as rib distension and tightness, tiredness and fatigue, heavy on head and body, depression and discomfort, dry and bitter in mouth, the difference was statistically significant(p<0.05);In group B,the quantitative scores of rib distension and tightness, tiredness and fatigue, heavy on head and body, depression and discomfort, dry and bitter in mouth, and Loose stool decreased significantly, and the difference was statistically significant(p<0.05);In group C,the quantitative scores of tiredness and fatigue, heavy on head and body, depression and discomfort, dry and bitter in mouth were obviously decreased, the difference is statistically significant(p<0.05).Compared with the three groups after treatment, the effect of group A was better than group B and better than group C in terms of tiredness and fatigue and depression and discomfort(p<0.05).(5)After 12 weeks of treatment, the liver fat parameters of group A and group B were lower than before, and the difference was statistically significant(p < 0.05).Comparison between groups suggested that group A was better than group B and better than group C.(6)Comparison before and after treatment in the same group: After 12 weeks of intervention, the weight,BMI,waist circumference, hip circumference,waist-to-hip ratio, and body fat rate of group A patients continued to decline, and the difference was statistically significant(p<0.05).The weight,BMI,waist circumference, hip circumference and waist hip of group B decreased before treatment, the difference was statistically significant(p<0.05),and the body fat rate decreased compared with before treatment, the difference was not statistically significant(p>0.05);The weight,BMI,waist circumference and hip circumference of group C decreased compared with before treatment, and the difference was statistically significant(p < 0.05).The waist-to-hip ratio and body fat rate decreased, but the difference was not statistically significant(p>0.05).Comparison between groups: the improvement of the body weight,BMI,waist circumference, hip circumference, body fat rate and other indicators of group A is better than group B and better than group C,the difference is statistically significant(p<0.05);There was no statistically significant difference(p>0.05);there was no statistically significant difference in body fat rate between group B and group C(p>0.05).(7)After 12 weeks of treatment, the levels of alanine aminotransferase and glutamine transpeptidase were reduced in the three groups compared with before, and the difference was statistically significant(p<0.05).Comparison between groups: The three groups had no statistically significant effect on serum alanine aminotransferase, aspartate aminotransferase and glutamine transpeptidase(p>0.05).(8)After 12 weeks of intervention, the total cholesterol and triglyceride contents of the three groups decreased significantly after treatment, and the difference was statistically significant compared with before treatment(p<0.05).Compared between groups, the improvement of total cholesterol and triglyceride in treatment group A after treatment was statistically different from that in groups B and C(p<0.05).The improvement of total cholesterol and triglyceride in group B was not statistically different from that in group C(p>0.05),suggests that group A is better than groups B and C in improving total cholesterol and triglycerides.(9)There were no serious adverse events and adverse reactions in the three groups of patients during treatment. There were no clinically significant changes in safety indicators such as vital signs, blood routine, urine routine, stool routine, renal function, and electrocardiogram.Conclusion:(1)Intermittent fasting therapy based on the Fuer Bigu theory can effectively reduce body weight, improve the degree of fatty liver grading, and improve the body morphology indicators such as BMI,waist circumference, hip circumference,waist-to-hip ratio, body fat rate and fat index of obese fatty liver patients.(2)Complete intermittent fasting for 2 days per week, and accumulatively complete intermittent fasting for 6 to 8 days per month for a total of three months, the effect is outstanding, which can effectively reduce weight and treat nonalcoholic fatty liver disease;accumulatively complete 4 to 6 days per month for a total of three months of intervention can also achieve a certain effect, but the effect is not good if it cannot be adhered regularly, which indicating that intermittent fasting therapy is a diet therapy that can be persisted for a long time.Taking the Wenyang Yiqi formula in the treatment of intermittent fasting can effectively improve the uncomfortable reactions such as starvation, fatigue and dizziness during fasting.
