Objective:By comparing the difference of curative effect,economy and prognosis between the patients before and after clinical pathway of the overall acupotomy treatment of cervical spondylosis,explore its evaluation method,and evaluate its clinical effect.To provide theoretical basis for the wide range promotion about clinical pathway of the overall acupotomy treatment of cervical spondylosis.Method:This study adopts the method of comparison before and after.Patients who were hospitalized from May 2017 to April 2018(before the implementation of the clinical pathway)and met the diagnostic criteria of cervical spondylosis were taken as the control group,patients who were hospitalized from May 2018 to April2019 and met the diagnostic criteria of cervical spondylosis and the inclusion criteria of this study were taken as the clinical pathway group.The control group was treated with acupuncture,acupotomy,medicine and massage,and the clinical pathway group was treated with the overall acupotomy treatment.The clinical efficacy,quantitative score,length of stay,cost of hospitalization,satisfaction,living condition and recurrence rate of the patients in the two groups were statistically analyzed by spss22.0 software.Results:1.General data: 192 patients were included in the study,including 92 in the clinical pathway group and 90 in the control group.There was no significant difference between the two groups in gender,age,clinical classification and other general data(P >0.05),which was comparable.2.Clinical effect: there were no cases of abscission in both groups.In the clinical pathway group,48 cases were cured,30 cases were effective,12 cases were effective,2 cases were ineffective,the total effective rate was 97.82%;in the control group,32 cases were cured,34 cases were effective,14 cases were effective,10 cases were ineffective,the total effective rate was 88.89%.There was significant difference between the two groups(P < 0.05).There was no significant difference between the clinical pathway group and the control group(P > 0.05)in the total effective rate of cervical spondylosis and radiculopathy(P < 0.05),while the clinical pathway group was significantly better than the control group(P < 0.05).3.Clinical quantification score: at the time of admission,the quantification score of clinical pathway group was 25.32 ± 2.18,while that of control group was 24.96 ± 2.54,with no significant difference(P > 0.05).At the time of discharge,the quantitative score of the patients in the clinical pathway group was 3.17 ± 1.68,and that in the control group was 7.84 ± 1.79,the difference was especlly significant(P < 0.05).4.Length of stay: the average length of stay in the clinical pathway group was 11.8days,and the average length of stay in the control group was 17.5 days,the difference between the two groups was statistically significant(P < 0.05).5.Hospitalization expenses: in terms of total hospitalization expenses,examination expenses and drug expenses,the clinical pathway group was significantly less than the control group,with a statistically significant difference(P < 0.05);while in terms of treatment expenses and daily average expenses,the difference between the two groups was equal.6.Inpatient satisfaction: the inpatient satisfaction of the clinical pathway group was 97.86 ± 1.88,and that of the control group was 89.25 ± 4.32,with statistical significance(P < 0.05).7.Rehospitalization rate: after half a year of treatment,there was no significant difference between the two groups(P > 0.05).After one year of treatment,two patients in the clinical pathway group were rehospitalized,severn patients in the control group,and the difference between the two groups was statistically significant(P < 0.05).8.Quality of life: after one year of treatment,the score of SF-36 in patients with clinical pathway was higher than that in the control group,and the difference between the two groups was statistically significant(P < 0.05).Conclusion:The clinical pathway of acupotomolysis for cervical spondylosis can significantly improve the clinical efficacy,improve the quality of medical treatment,shorten the length of stay,reduce the cost of medical treatment,save medical resources,improve the satisfaction of patients in hospital,and improve the prognosis and quality of life of patients.Therefore,it has a good clinical application prospect and is worth popularizing in a wide range.
