Clinical Observation Of American Chiropractic Combined With Acupotomy In Treatment Of Cervical Spondylotic Radiculopathy

ObjectiveBased on the American Chiropractic,the clinical effect of American Chiropractic combined with acupotomy on the patients with cervical spondylosis radiculopathy was observed.The differences of the two groups in pain symptoms and dysfunction degree was compared,which provided a preliminary reference for the clinical exploration and optimization in treatment for cervical spondylosis radiculopathy.MethodThis study is an observational study.From March 2020 to January 2021,the patients with cervical spondylosis radiculopathy who were treated in the department of acupuncture and orthopedic of the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine were divided into two groups according to the patients’willingness.Respectively,the patients of observation group receive the American chiropractic combined with acupotomy,and the patients of control group receive the single American Chiropractic.The patients of two groups all were treated three times,once a week.The trained personnel are responsible for the information collection and scale evaluation of the included patients,that is,the general situation of patients before treatment such as gender,age,course,condition(VAS and NDI)were collected.After treatment and one month after treatment,the scales of Visual Analogue Scale(VAS)and Neck Disability Index(NDI)were evaluated again.The general situation and scale score of the two groups were statistically analyzed by SPSS22.0 software.First,the two groups were compared to determine whether the baseline of the two groups was comparable.If there was no comparability,the baseline matching of two groups was performed by the propensity score match(PSM),that is,the prognosis of cervical spondylotic radiculopathy was taken as the dependent variable,and other factors that may affect the clinical efficacy such as gender,age,course and condition were independent variables to construct logistic regression model.The tendency score of each individual sample was calculated.The calipers value was set in the range of 0.01~0.2 by using the nearest matching method of 1:1.Finally,the patients of two groups who matched successfully were compared statistically,that is,the pain relief and functional improvement were compared and analyzed according to the data of two scales evaluated three times.Result81 patients with cervical spondylosis radiculopathy were collected,35 patients in observation group and 46 patients in control group.Because the baseline between the two groups was not comparable,the PSM method was used to balance the differences between the two groups.When the calipers value was set to 0.1,30 pairs of samples were successfully matched,namely each 30 cases in the observation group and the control group.1.Baseline comparison between the two groupsBefore matching,there were no statistical significance between the observation group and the control group in gender,course and condition(pain degree),and there were statistical significance in age,condition(cervical dysfunction degree)(Page<0.05,PNDI<0.05).After matching,the baseline data between the two groups was not statistically significant(P>0.05).2.Comparison of clinical efficacy between the two groupsThere were 30 patients in the observation group,including 15 cases cured,8 cases markedly effective,5 cases improved and 2 cases ineffective.The rate of clinical effect was93.3%.There were 30 patients in the control group,including 6 cases cured,8 cases markedly effective,8 cases improved and 8 cases ineffective.The rate of clinical effect was 73.3%.The rate of clinical effect of the observation group was 93.3%,which was significantly higher than73.3%of the control group(P<0.05).3.Comparison of VAS scores between the two groupsAccording to the data of VAS before treatment,after treatment and one month after treatment,the VAS scores of the two groups all were decreased gradually,and the difference was statistically significant(P<0.05).Comparison between the two groups:Before treatment,the difference between the two groups was not statistically significant(P>0.05).After treatment,the VAS score of the observation group was significantly lower than that of the control group(P<0.05).One month after treatment,the VAS score of the observation group was also significantly lower than that of the control group(P<0.05).4.Comparison of NDI scores between the two groupsAccording to the data of NDI before treatment,after treatment and one month after treatment,the NDI scores of the two groups all were decreased gradually,and the difference was statistically significant(P<0.05).Comparison between the two groups:Before treatment,there was no significant difference in NDI scores between the two groups(P>0.05).After treatment,the NDI score of the observation group was significantly lower than that of the control group(P<0.05).One month after treatment,the NDI score of the observation group was also significantly lower than that of the control group(P<0.05).ConclusionAccording to this study,based on the American Chiropractic,acupotomy can improve the clinical efficiency,achieve faster effect,alleviate the pain and improve the cervical function more obviously for patients with cervical spondylosis radiculopathy.There was no serious adverse reactions occurring.
