ObjectivesTo observe the effect of massage on the function characteristics of Meridians of Zuyangming in patients with knee osteoarthritis.And to provide a basis for the role of "meridians theory" in the prevention and treatment of KOA,so as to improve the clinical efficacy of massage in the treatment of knee osteoarthritis.MethodsAll cases are from the department of orthopedics and massage department of Wuxi Hospital of Nanjing Traditional Chinese Medicine University.The 75 patients who met the inclusion criteria and exclusion criteria were divided into three groups by random number table method:in group A,25 patients were treated with quadriceps femoris functional exercise;in group B,25 patients were treated with oral nonsteroidal antiinflammatory drugs combined with quadriceps femoris functional exercise;25 cases of group C,massage combined with quadriceps functional exercise.The three groups of KOA patients were graded by Kellgren Lawrence(K-L)X-ray grading,and the treatment cycle was 2 weeks.The results were evaluated with WOMAC scale score,Lequesne index score,gait analysis and quadriceps muscle tension.Result1.A total of 10 patients were unable to complete observer due to lack of cooperation in treatment or other treatment methods during follow-up,including 5 cases in group A,3 cases in group B and 2 cases in group C.Finally,20 cases in group A,22 cases in group B and 23 cases in group C completed the study and obtained follow-up.After examination and comparison,there were no significant statistical differences in the baseline data of male to female ratio,affected knee position,age and course of disease among the three groups(P>0.05),suggesting that the baseline data of the three groups were comparable.2.WOMAC scale score:?Compared with WOMAC score before treatment,there was no significant difference among the three groups(P>0.05);?The scores of pain,functional activity and comprehensive score after treatment have varying degrees of reduction compared with those before treatment,and the differences were statistically significant(P<0.05);the stiffness score of B group and C group was statistically significant compared with those before treatment(P<0.05),while the difference in stiffness score in group A after treatment was not significant compared with that before treatment(P>0.05).?After treatment,the pain score of group B and group C reduced significantly compared with that of group A(P<0.05),while the pain score of group B improved significantly compared with that of group C(P<0.05).There was statistically significant difference in stiffness score between group B and group C after treatment compared with group A(P<0.05),while in stiffness score,there was no significant improvement among group B and group C after treatment(P>0.05).The functional activity scores of group B and group C decreased significantly after treatment compared with group A(P<0.05).However,compared with group B and group C after treatment,there was no significant improvement in functional activity score(P>0.05).In terms of comprehensive scores,compared with group A,the comprehensive scores of group B and group C improved significantly after treatment(P<0.05).Compared with group B,there was no significant difference in the comprehensive score of group C after treatment(P>0.05).3.Lequesne index score:?There was no statistical difference in the scores of Lequesne index among the three groups before treatment(P>0.05);?The Lequesne score decreased after treatment compared with that before treatment,and there was a statistically significant difference(P<0.05).?After treatment,the Lequesne score in group B and group C decreased significantly compared with that in group A(P<0.05).Compared with group B,there was no significant difference in the improvement of Lequesne score in group C(P<0.05).4.Analysis of gait parameters:?There was no significant difference in gait parameters between the three groups before treatment(P>0.05):?Comparison of the three groups before and after treatment showed that the step size and step speed of the three groups after treatment were increased compared with that before treatment,and the difference was statistically significant(P<0.05);The double support phase parameters of the three groups showed different degrees of decrease compared with those before treatment(P<0.05);?After treatment,compared with group A,the step size of group B and group C increased significantly after treatment(P<0.05),and the improvement of group C was more significant than that of group B(P<0.05).In terms of step speed parameters,there was a statistically significant difference between group B and group C after treatment and group A(P<0.05),while there was no significant difference in walking speed among group B and group C after treatment(P>0.05).In terms of the parameters of the double support phase,the improvement of group B and group C after treatment was more significant than that of group A(P<0.05),while the difference between group C and group B was not statistically significant(P>0.05).5.Quadriceps muscle tension detection:?Before treatment,the quadriceps muscle pressure(L500g)displacement value on the affected side of the three groups,namely quadriceps muscle tension,was compared in pairs,and the difference was not statistically significant(P>0.05);?Comparison of the three groups before and after treatment,the displacement value of L500g of the quadriceps femoris of the patients reduced after treatment,the difference was statistically significant(P<0.05);?After treatment,the L500g displacement of quadriceps genicus in group B and group C was significantly reduced compared with that in group A(P<0.05).Compared with group B,the improvement of L500g displacement of quadriceps femoris in group C was more significant,with a statistically significant difference(P<0.05).Conclusion1.The quadriceps muscle functional exercise group,the oral nonsteroidal antiinflammatory drugs group combined with quadriceps muscle functional exercise group,and the manipulative massage combined with quadriceps muscle functional exercise group all had certain clinical effects on KOA patients,all of which could improve the WOMAC score,Lequesne index,gait parameters and quadriceps muscle tension of the patients,and improve the walking ability of the patients.2.Oral nonsteroidal antiinflammatory drugs combined with functional exercise or gimmick massage the quadriceps muscle and functional exercise to treat KOA,the clinical curative effect is superior to the pure functional exercise,while gimmick massage the quadriceps muscle and functional exercise quadriceps functional status improvement is greater than the oral nonsteroidal antiinflammatory drugs combined with functional exercise.3.Per unit time of KOA patients suffering from knee femoral quadriceps muscle group lines of massage treatment,use "Gun" method is given priority to,can better improve the resting state of KOA patients quadriceps muscle tension,so as to improve the quadriceps functional status:the function of Foot-Yangming meridian-muscle,enhance the effect of the "bundle of bone and joint".
