Purpose:To observe the clinical effect of blood stasis syndrome of lumbar disk herniation treatment based on extracorporeal shock wave treatment along the meridians combined with acupuncture“Five acupoint"Methods:60 patients with lumbar disc herniation who met the criteria were randomly divided into shock wave group,acupuncture group and combined group with 20 cases each.The shock wave group was treated with extracorporeal shockwave alone,the acupuncture group was treated with acupuncture "Five acupoint" alone,and the combined group was treated with the above two methods.Once every 3 days,4 times as a course of treatment,a total of 12 days.By observing the patients with Visual analog pain score(Visual Analogue Scale,VAS),Oswestry disability index(ODI score),the Japanese Orthopaedic Association(Japanese Orthopaedic Association,JOA)score,standard of effect in<guide principles of clinical research on the treatment of lumbar disc herniation with new Chinese medicine>to evaluate the efficacy.At the same time observe the adverse reaction and prognosis of three groups.Results:1?The general data,VAS score,ODI score and JOA score of the three groups before treatment showed no significant difference after statistical analysis(P>0.05),indicating comparability.2?VAS scores in the three groups after treatment and before treatment were all decreased,with statistically significant difference(P<0.05).The VAS score of the combined group was lower than that of the shock wave group,and that of the shock wave group was lower than that of the acupuncture group after treatment,with statistically significant difference(P<0.05).The VAS score before each treatment in the shockwave group and the combination group increased slightly compared with that after the last treatment,but the overall score after treatment showed a downward trend,and the increase rate before the second and third treatment in the combined group was lower than that in the shockwave group.3?The ODI scores of the three groups were decreased after treatment and before treatment,with statistically significant difference(P<0.05).After treatment,the ODI score of the combined group was lower than that of the shockwave group,and that of the shockwave group was lower than that of the acupuncture group,with statistically significant difference(P<0.05).4?The JOA scores of the three groups increased after treatment and before treatment,with statistically significant difference(P<0.05).After treatment,the difference between the acupuncture group and the other two groups was statistically significant(P<0.05).However,there was no significant difference between the combined group and the shockwave group(P>0.05).During the treatment,there was no statistically sig9ificant difference in JOA score increase between the combined group and the shockwave group after the first and fourth treatment(P>0.05),but there was a statistically significant difference in JOA score increase after the second and third treatment(P<0.05).5?There was no significant difference in effective rate among the three groups(P>0.05).Compared with the other two groups,the acupuncture group had the lowest healing rate(P<0.05).However,there was no significant difference between the combined group and the shock wave group(P>0.05).Patients in the three groups who recovered after one month of treatment were followed up without recurrence.6?There was no significant difiference in the incidence of adverse reactions among the three groups(P>0.05).Conclusion:1?After shock wave combined with acupuncture treatment,JOA score improved to a similar degree as that of shock wave treatment alone,but the improvement rate was better than that of shock wave treatment alone.2?Shock wave does have significant immediate analgesic effect and long-term cumulative effect.Combined acupuncture treatment can help expand the advantage of shock wave and relieve the pain recurrence of shock wave to some extent during the treatment.3?All the methods in the three groups are safe and effective,which can effectively alleviate the pain of patients,improve the symptoms and lumbar function of patients,and improve the daily living ability of patients.However,the combined group has the best effect,which is worthy of clinical promotion due to its simple and convenient operation and short treatment time.
