Research background and purposeCerebral infarction refers to a variety of causes of brain tissue hypoperfusion,resulting in neurological deficit lesions,which is the most common type of stroke in the clinical,accounting for about 60-80% of all stroke.Cerebral infarction,with a high incidence,mortality and disability rate,has brought huge economic burden to families and society.With the development of imaging technology,especially magnetic resonance technology and clinical application of new drugs,the diagnosis and treatment of acute cerebral infarction.The prognosis has been greatly improved,but the ability to screen for high-risk groups needs to be improved,in addition to some of the clear risk factors for cerebral infarction,such as hypertension,diabetes,hyperlipidemia,hematopathy,heart disease,alcohol and tobacco history etc.It also urgently need to find new risk factors to better reduce the incidence of cerebral infarction.The perivascular space is a potential lacuna between the cerebral blood vessels and the brain parenchyma.It is a normal anatomical structure in the brain and has certain physiological and immune functions.Under normal circumstances,the gap diameter is less than 2mm,and when the diameter is more than 3mm,it is called an enlarged virchow robin space.More and more studies have shown that EVRS is closely related to diseases such as demyelinating diseases,degenerative diseases,infectious diseases,migraine,and epilepsy in the nervous system.In recent years,more and more research shows that the perivascular space is closely related to the incidence of cerebral infarction,and different regions of the EVRS have different effects on cerebral infarction.This research is to study the relationship between EVRS in the different regions and the incidence of cerebral infarction by Meta-analysis.Research methodsSearch terms were identified with "cerebral infarction" and "perivascular space".The search languages were Chinese and English.The English database include pubmed,SCI database,Embase database,Chinese database include Wanfang Database,China National Knowledge Infrastructure(CNKI),and Condition Based Maintenance(CBM),which conducts topic and full-text search in selected databases,uses the endnote software and manual retrieving of duplicate documents,and reads abstracts and full-text screening of documents,based on established inclusion and exclusion Standards,removal of unsatisfied literature,application of NOS scores.Select a document with a score greater than 5 points and finally obtain 7 documents that meet the quality requirements.STATA software was used for Meta-analysis.Based on the heterogeneity test results,the fixed-effect model or random-effect model was used to calculate the combined OR value and its 95% confidence interval.The sensitivity analysis was used to determine the stability of the results.Egger's and funnel plots analyze publication bias.Research result1.A total of 7 articles were included.Meta-analysis showed that more than 10 EVRS in the basal ganglia region were significantly associated with the onset of cerebral infarction,with a combined OR=1.50(95%CI: 1.26,1.74),an increased perivascular space may increase the risk of cerebral infarction.,heterogeneity test,sensitivity analysis can be accepted,there is no obvious publication bias;2.Meta-analysis of more than 20 EVRS and cerebral infarction in the basal ganglia region revealed that the perivascular space was significantly associated with the incidence of cerebral infarction.The combined OR=0.62(95% CI: 0.28,0.96),but the degree was not as mild of expanded perivascular space;3.There was no significant correlation between more than 10 EVRS in centrum semiovale and the incidence of cerebral infarction 0.09(95% CI:-0.13,0.30).Conclusion1.The EVRS in the basal ganglia is a risk factor for the development of cerebral infarction.However,the increased perivascular space does not increase the risk of cerebral infarction.2.There was no significant correlation between EVRS in the centrum semiovale and the incidence of cerebral infarction;...
