Objective:To investigate the distribution,signal and enhancement characteristics of the the M1-segment of middle cerebral artery(MCA)plaque in patients with acute ischemic cerebral infarction in basal ganglia and non-basal ganglia regions use the high resolution magnetic resonance angiography(HR-MRI)plain scan and enhanced sequence.Methods:From October 2018 to June 2019,62 patients with acute cerebral infarction of middle cerebral artery territory were collected in Baoding First Central Hospital.All patients underwent the MRA,plain and enhanced T1W-3D-VISTA.All patients were divided into basal ganglia group and non-basal ganglia group.The distribution,the plaque-wall contrast ratio(CR),contrast enhancement ratio(ER)and the characteristics of enhancement pattern of plaque in M1-segment of MCA were compared between the two groups.Results:1.There were 62 cases of acute MCA cerebral infarction,including 36 patients in basal ganglia group and 26 patients in non-basal ganglia group,and the difference in baseline data between the two groups was not statistically significant(P > 0.05).2.A total of 878 images(basal ganglia group: 534,the non-basal ganglia group: 344)contains blood vessels plaques,and were frequently located at the ventral(31.4%)wall.In the basal ganglia group,the plaque was commonly located at the superior(34.0%)wall,followed by the ventral(27.1%)wall,with a smaller proportion of the inferior(21.0%)wall and dorsal(17.9%)wall,while in the non-ganglia group,it was commonly located at the ventral(37.4%)wall,followed by the inferior(30.3%)wall,with a smaller proportion of the dorsal(18.8%)wall and the superior(13.5%)wall.The difference in plaque location distribution between the two groups was statistically significant(?2 = 92.919,P < 0.001).3.Both basal ganglia group and non-basal ganglia group have more high signal in T1WI(?2 =2.088,P=0.148).The CR of the basal ganglia group was lower than that of the non-basal ganglia group(1.51(1.44?1.60)vs 1.63(1.51?1.67),Z=-2.669,P=0.008).4.The grade 2 enhancement pattern of ER was common in basal ganglia group and non-basal ganglia group(Z=-0.744,P=0.457).However,the ER value of basal ganglia group was lower(2.22(1.77?2.44)vs 2.54(1.92?2.64),Z=-2.247,P=0.025).The proportion of plaque type 2 enhancement pattern in the basal ganglia group was lower than that in the non-basal ganglia group,and the difference was statistically significant(?2 =18.572,P < 0.001).Conclusion:In patients with cerebral infarction in the blood supply area of M1-segment of MCA,the plaques were located at the ventral wall commonly.And in the basal ganglia group,the plaques were commonly located at the superior wall,followed by the ventral wall,while in the non-basal ganglia group,the plaques were commonly located at the ventral wall,followed by the inferior wall.The plaques in basal ganglia group and non-basal ganglia group are unstable,and the stability of plaques in basal ganglia group is higher than that in non-basal ganglia group.HR-MRI is a good technique for the analysis of vascular plaque characteristics in M1-segment of MCA in acute cerebral infarction in basal ganglia region.
