Research BackgroundVoluntary blood donation refers to the act of selflessly dedicating blood to others who use blood in order to save the lives and health.Blood is of great significance in restoring the lives of patients and promoting the recovery of health. In order to encourage people to participate in voluntary blood donation, standardize and guide the development of non-remunerated blood donation, the State issued the"Blood Donation Law of the People's Republic of China" on October 1,1998,(hereinafter referred to as "Blood Donation Law").Article 2 of the law stipulates that the State institutes the system of blood donation without compensation, that is,China has implemented a system of voluntary blood donation since October 1,1998.The blood sources for clinical blood use will all come from non-remunerated blood donors. At present, most countries in the world have implemented voluntary blood donation systems. In 2013,the World Health Organization(WHO)Blood Safety Database pointed out that 100%of the blood supply in 57 countries worldwide comes from voluntary blood donors. With the development of economy and the progress of medical level, the clinical blood volume in China is growing at a rate of 10%-15%per year, and the amount of voluntary blood donation can no longer meet the increasing blood demand for clinical blood use.At the same time,China has a weak foundation for voluntary blood donation,a lack of awareness of citizens' voluntary blood donation, and a low rate of voluntary blood donation.Recruiting more voluntary blood donors to participate in blood donation to meet the needs of clinical blood has become the primary task of blood collection and supply institutions. As the only blood supply institution in Jinan City,Jinan Blood Supply and Insurance Center undertakes the clinical blood supply task of more than 70 municipal hospitals, county hospitals, factory hospitals, private hospitals and 11 provincial hospitals within the jurisdiction of Jinan City.As the clinical blood supply continues to rise, the clinical blood supply in Jinan is in a state of tension for a long time.In recent years, college students have gradually become the main force of voluntary blood donation.Referring to the software of Jinan Blood Supply Center,it was found that in the past five years, the annual blood donation of college students in Jinan accounted for 23.71%-32.10%of the total blood collection, ranking firs among the total annual blood donors. However, due to the large number of college students, there is still a large potential to be tapped. Therefore,it is of great significance to understand the present situation of voluntary blood donation among college students in Jinan area and factors affecting the cognition, willingness and behavior of voluntary blood donation,so as to formulate corresponding countermeasures to recruit more college students to participate in blood donation and to promote the development of voluntary blood donation among college students.Research methodsThis study conducted an anonymous self-assessment questionnaire survey in three universities in Jinan, and this is a cross-sectional study with a stratified cluster sampling design.Collect the general data of the subjects, knowledge, attitude and behavior of voluntary blood donation, etc. Then explore the knowledge, attitude, and behavior of voluntary blood donation among college students in Jinan city and its influencing factors through univariate and multivariate analysis. The analyses were performed in the Epidata software version 3.0 and SPSS software version24.0.Research result1.A total of 1400 questionnaires were collected from this research, and 1388 valid questionnaires were collected. The effective rate of questionnaire collection was 99.14%.The survey targets college students from three universities in Jinan. The ratio of medical major to non-medical major is 1.13:1,and the age is between 18 and 38 years old.2.The average score of 1388 respondents on blood donation related knowledge was 6.31±2.22,and the overall awareness rate was 57.36%.The major and blood donation behavior was the influencing factor of blood donation-related knowledge scores(P<0.05),and the scores of medical majors and students with blood donation behaviors were higher.3.In this study,978(70.46%)responders had the willingness to participate in voluntary blood donation, and 410(29.54%)responders had no intention.Major, grade, knowledge of blood donation, whether to encourage people to participate in blood donation, family support and opinion on blood station are influencing factors of college students' blood donation willingness.Among which medical major, lower grade, high level of knowledge about blood donation, willingness to encourage others to participate in blood donation and family support contribute to the willingness to blood donation(P<0.05).Gender has no effect on the willingness to donate blood.4.In this study, there were 322 students with blood donation behavior,1060 students without blood donation behavior and 6 data was missing. The blood donation rate was 23.20%.And most of them are satisfied with the blood donation process(84.47%).Those who donate blood are more inclined to participate in regular blood donation, encourage others to participate in blood donation and participate in volunteer services and publicity activities.(P<0.05).In the multivariate logistic regression model,males(OR=2.13,95%CI:1.58-2.87,P<0.001),medical students(OR=1.44,95%CI:1.05-1.99,P=0.026),owning high level of blood donation knowledge(OR=2.02,95%CI:1.39-2.94,P<0.001)and willingness to donate blood(OR=8.67,95%CI:5.24?14.36,P<0.001),and friends around us had the donated blood activity(OR=1.75,95%CI:1.01?3.07,P<0.049)were predictors of blood donation behavior,While the blood donation behavior of the lower grades students were relatively inactivity compare with high grade students(OR-0.35,95%CI:0.26-0.49,P<0.001).Conclusions1.The overall awareness rate of the knowledge of voluntary blood donation among college students in Jinan was relatively high, and the scores of medical students and students with blood donation behavior were higher.2.The willingness of college students in Jinan to participate in voluntary blood donation was at a high level.Medical students, lower grade students, students with more knowledge of blood donation and students whose parents support voluntary blood donation were more inclined to donate blood.3.Among college students in Jinan, males, seniors, medical students, students whose friends had donated blood, and students with higher level of knowledge about blood donation were all more likely to donate blood. The experience of blood donation, such as environmental comfort, attitude of the staff, skill of the staff, whether the souvenir is attractive, was also an important factor affecting the behavior of blood donation.Policy SuggestionsWe should strengthen the publicity and education of college students' knowledge of voluntary blood donation, enrich the forms of publicity and education, and improve the effect of publicity and education on blood donation.Meanwhile,we should establish a long-term mechanism with the government as the leading role and the whole society participating in blood donation,so that we can form a social atmosphere of actively participating in blood donation for college students.By improving the professional skills and service attitude of the staff of blood collection and supply institutions, improving the blood donation environment, providing psychological support for college students' blood donors, and providing attractive souvenirs, the college students will be given a good blood donation experience and the voluntary blood donation rate of college students will be improved.
