Objective:?To observe the expression of visfatin and retinol binding protein 4 in different levels and positions of placenta in normal pregnancy and hypertancy disorder of pregnancy(HDP),To investigate the role of visfatin and retinol binding protein 4 expression in placental tissue in the pathogenesis of HDP and its predictive value for HDP.Method:84 cases of hypertensive disorder complicating pregnancy in the First Affiliated Hospital of Kunming Medical University as experimental group,Among them,24 cases of patients with hypertensive pregnancy,28 cases of patients with early-onset preeclampsia,28 cases of patients with late-onset preeclampsia;during the same period,60 normal full-term pregnant women as control group.All the samples were taken within 30 minutes after the delivery of the placenta.Avoid haemorrhage,infarction,and calcification in the naked eye,Centering the attachment of the umbilical cord,Two placentas of 1*1cm size were taken at the edge of the umbilical cord and between the two sides,The three placental tissues were divided into upper,middle and lower layers,that is,maternal,chorionic and fetal.,Each piece is 1*1*1cm in size and placed in nine cryopreserved tubes for sterilizing enzymes,Mark the name,position,group on the cryopreservation tube and quickly transfer to the-80 refrigerator for storage to be tested.The remaining placenta was fixed with paraformaldehyde,dehydrated and embedded for immunohistochemical examination.Immunohistochemical method was used to detect the expression of visfatin and retinol binding protein 4 in placenta of normal pregnancy and hypertensive disorder of pregnancy,Conventional microwave pretreatment,dewaxing and hydration of paraffin sections.hydrogen peroxide was incubated at room temperature for 10 min.Then the experiment was carried out according to the sabc reagent specification.Immunohistochemical images were processed with imag-j image processor and the average optical density was measured.Enzyme-linked immunosorbent assay was used to detect endolipin and retinol binding protein in placenta homogenate of different levels in experimental group and control group,The determination method is carried out strictly according to the instruction,The od value was read by enzyme labeling instrument.The data were analyzed by SPSS21.0 software.The measurement data are expressed as mean standard deviation(xs).Sample t-test was used to compare the two groups.Single factor analysis of variance(ANOVA)was used in the multigroup comparison.The correlation between the two factors and blood pressure was analyzed by person correlation.Results:The results of immunohistochemistry showed that the expression of visfatin and retinol binding protein in placenta of hypertensive disorder of pregnancy group and control group was different in different positions and levels.The positive sections of visfatin and RBP4 were brown.Under the high-power microscope,it was brown and yellow,In the cytoplasm of trophoblastic cells and syncytiotrophoblastic cells,Immunohistochemical staining was observed in all sections except the negative control group.The expression of visfatin and RBP4 in early preeclampsia group was more than that in the control group.Strong reaction intensity;Late preeclampsia followed;GH group was weaker than late onset group.The weakest is the normal group.Average optical density analysis visfatin:Early onset 0.420.13 vs late onset 0.33 0.08 vs GH 0.27 0.20 vs normal 0.140.03,p<0.05.RBP4:early onset 0.550.37 vs late onset 0.410.20 vs PIH 0.21 0.14 vs normal 0.14 0.01.Comparison of horizontal levels at 9 points:there is a statistically significant difference in the marginal surface of the middle surface of the umbilical cord surface(p<0.05).There was no significant difference between fetal plane and middle plane in vertical plane(p>0.05).There was no significant difference in maternal surface>fetal surface p<0.05.Elisa was used to measure rbp4 concentration in placenta of hypertensive complicating pregnancy group and control group.The visfatin concentration of early onset group was:late onset group:gestational hypertension group:normal group:Data showed that the concentration of RBP4 in early onset group>late onset group>PIH group>normal group,p>0.05,the difference was statistically significant.Comparison of mean Water level in 9 points:umbilical Cord>Interfacial>marginal plane p<0.05 difference is statistically significant in vertical plane comparison by fetal plane and midplane value,p>0.05 has no statistical significance.The difference of maternal>fetal(p<0.05)was statistically significant.?There is no correlation between the expression of visfatin and retinol binding protein in placenta of HDCP patients.? There was a positive correlation between visfatin,RBP4 and blood pressure,and the correlation coefficients were:Conclussion:?compared with normal women,the levels of visfatin and retinol binding protein in placenta of hypertensive disorder complicating pregnancy were significantly increased?In normal group,gestational hypertensive(GH)group and preeclampsia group,the expression of endolipin and retinol binding protein in placenta was different in different locations,and the expression of retinol binding protein was different in different levels of placenta.These results suggest that placental heterogeneity has an effect on the expression of mesolipin and retinol binding protein.?The expression of endothelin and retinol binding protein may be involved in the pathogenesis of hdcp,but there is no correlation between them in placenta.
