A Study On The Correlation Between LIF, IL-1β In Placenta And The Hypertensive Disorder Complicating Pregnancy

Objective:①To evaluate the changes of the levels of LIF, IL-1βin placenta of patients with hypertensive disorder complicating pregnancy.②To evaluate the correlation between the abnormality of the levels of LIF and IL-1βin placenta and the morbidity of hypertensive disorder complicating pregnancy.③To evaluate the correlation between the levels of LIF and IL-1βin placenta and the degree of severity of hypertensive disorder complicating pregnancy.Methods:1 The study subject1.1 The study group: 15 patients with gestational hypertension (gestational hypertension group) who had prenatal examinations and delivered in the Obstetrics of the Second Hospital of HeBei Medical University were selected from December 2005 to April 2006. Mean age of them was 28.15±1.77 years old, mean gestational weeks of them was 38.44±0.99 weeks. Diagnosis criteria: blood pressure≥140/90mmHg, which happen in pregnancy for the first time, and can recover after 12 weeks of delivery; proteinuria(-); the patients can be accompanied with epigastric discomfort or thrombocytopenia, which only can be diagnosed after delivery. 30 patients with preeclampsia (PE group, 18 patients with severe preeclampsia and 12 patients with mild preeclampsia were involved). Mean age of them was 27.60±3.31 years old, mean gestational weeks of them was37.13±2.61 weeks. Diagnosis criteria of severe preeclampsia were as follows: blood pressure≥160/110mmHg; proteinuria≥2.0g/24 hours or proteinuria of (++); serum creatinine>106umol/L; platelet count<100×109/L; microangiothic hemolysis (elevation of serum LDH); elevation of serum ALT or AST; persistent headache or other disturbances of cranial nerve or vision; persistent discomfort of the epigastrium. Diagnosis criteria of mild preeclampsia were as follows: blood pressure≥140/90mmHg after 20 weeks of gestation; proteinuria≥300mg/24 hours or proteinuria of (+); accompanying of symptoms of epigastric discomfort or headache. 1.2 The control group: 15 healthy third trimester pregnant women were selected to serve as the control group. Mean age of them was 26.87±2.00 years old; Mean gestational weeks of them was 38.60±1.03 weeks. All the subjects must meet the following criteria: primipara, no multi gestations, no chronic hypertension, no diabetes, no heart disease, no chronic renal disease, no chronic liver disease and no immune disease.2 Sample collecting: About 1cm3 placental tissues were collected from the pregnant women after the placenta was deliveried. Use axenic 0.9% sodium chloride to wash it and fix it with formaldehyde. 3 The experiment method: Use immunohistochemical method to determine the levels of LIF and IL-1βin placenta.Results:1 The exression position of LIF and IL-1βin placenta: they were all expressed in syneytiotrophoblast.2 The expression and comparision of LIF in the study and control group in placenta: the expression in the study group was markedly lower than that in the control group, there were remarkably statistical differences when the PE group compared with gestational hypertension and control group, but there was no statistical difference between gestational hypertension group and control group.3 The expression and comparision of IL-1βin the study and control group in placenta: the expression in the study group was markedly higher than that in the control group, there were remarkably statistical differences when the PE group compared with gestational hypertension and control group, but there was no statistical difference between gestational hypertension group and control group.4 The correlation between LIF and IL-1β: there was a negtive correlation between the expression of LIF and IL-1β.5 The correlation between LIF, IL-1βand mean arterial pressure(MAP) and proteinuria(24hours): LIF correlated negtively with the levels of MAP and proteinuria(24hours), IL-1βcorrelated positively with the levels of MAP and proteinuria(24hours). Conclusions:1 In the study we found that there was no statistical difference about the expression of LIF, IL-1βin placenta when the gestational hypertension group compared with the control group, we presumed that gestational hypertension should be an intermediate step from healthy pregnancy to PE, the patients may be in the initial stage of the disease, and the body may have partly adjustment.2 There was a negative correlation between the expression of LIF and IL-1β, which may influence the expression of IL-1βto promote the damage of the endothelial cell to lead to hypertensive disorder complicating pregnancy.3 The abnormal expression of LIF and IL-1βwere more severity, accompanied with the progress of the disease, which will be a new monitoring of the degree of hypertensive disorder complicating pregnancy.4 The abnormal expression of LIF and IL-1βin placental tissue in hypertensive disorder complicating pregnancy may influnce the development of the blood vessel of placenta by influnced the corrasion of trophocyte, which may participate in the pathogenesis of hypertensive disorder complicating pregnancy. The man-made inducement of LIF or interdiction of IL-1βmay control the occurrence and progress of hypertensive disorder complicating pregnancy, which may be a new assistant method to treat hypertensive disorder complicating pregnancy.
