Objective To understand the Disposition of Time Management and the status quo of Clinical Practice Behavior of nursing students in the early,middle and late period of practice,to compare the changes of the two,to explore the influence of time management on clinical practice behavior,and to analyze the factors affecting clinical practice behavior.Methods This paper is divided into two parts: quantitative research and qualitative research.1.Quantitative research:the convenience sampling method is used to select 100 subjects from the 2016 level higher vocational nursing students of Nursing Department of a higher vocational college to the interns of provincial,municipal and county-level hospitals,which meet the exclusion criteria.In the early stage(thesecond month of practice),the middle stage(the fifth month of practice),the end stage(the eighth month of practice),the youth time management tendency scale and the clinical practice behavior scale are used The self-made demographic data questionnaire was conducted for three times.The data were recorded in SPSS 22.0software,and were analyzed by descriptive statistics,multiple factor ANOVA of repeated measurements,Pearson correlation analysis and regression analysis of generalized estimation equation,all of which were statistically significant with P ?0.05.2.Qualitative research:according to the results of quantitative research,semi-structured interview is used to deeply understand the real experience of clinical practice of vocational practice nursing students.Based on the principle of information saturation,interview the random objects,sort out the feelings of the objects for practice,and sort out the interview results.The interview results of 16 vocational practice nurses can obtain all the information,so The subjects were selected from the16 practical nurses and extracted by the seven step analysis of Colaizzi phenomenological data.Results1.The total average scores of time management disposition of higher vocational nursing students in different periods were(3.74 ± 0.48),(3.47 ± 0.33),(3.35 ± 0.68),and the scores of each dimension from high to low were time value,time efficacy and time monitoring.It can be seen that with the increase of practice time,the scores of each dimension items have a downward trend,so the total scores also show a downward trend.2.The total average scores of clinical practice behavior items in different periods of practice of higher vocational nursing students are(4.02 ± 0.62),(4.18 ± 0.55),(4.06 ± 0.55),which are in the upper middle level,but with the increase of practice time,the total scores first rise and then fall.3.There was a significant difference in the total average score of time management disposition among different genders(girls > boys),places of origin(cities > countryside),students cadres(students cadres > non students cadres),parents with the highest education background(P < 0.05),period and gender interaction effect(P < 0.05),period and place of origin interaction effect(P < 0.05),period and study nursing The interaction effect of the reason was significant(P < 0.05).4.There was a significant difference in the total average score of clinical practice behavior among the students from different places of origin(Urban > rural),the level of practice hospital(provincial > municipal > County),and the highest degree of parents(P < 0.05).The interaction effect of period and different demographic variables on clinical practice behavior should not be significant(P >0.05).5.There was a positive correlation between early time management tendency and early clinical practice behavior(r = 0.52,P < 0.01);there was a positive correlation between middle time management tendency and middle clinical practice behavior(r = 0.476,P < 0.01);there was a positive correlation between late time management tendency and late clinical practice behavior(r = 0.747,P < 0.01).6.Taking the total score of clinical practice behavior as dependent variable,gender,whether only child,place of origin,hospital level,student cadre,average academic achievement,reasons for studying,and parents' highest education as independent variables,and taking three dimensions of time management items: sense of time value,sense of time monitoring and sense of time efficacy as covariates,a generalized estimation equation model regression analysis was established.The results showed that the influencing factors of clinical practice behavior included the source of students,practice hospital,parents' highest education,sense of time value,sense of time monitoring and sense of time efficacy.Taking the city as the reference group,the rural area as the risk factor,the practice hospital in the county hospital as the reference group,the practice hospital at the city level(= 0.135,P = 0.012)and the provincial level(= 0.195,P < 0.001)were the protective factors.The highest education level of the parents was taken as the reference group,and the parents' Education in senior high school(=-0.133,P = 0.003)and junior high school(=-0.221),P < 0.001)was a risk factor for the total score of clinical practice behavior?The three dimensions of time management: time value(= 0.124,P < 0.001),time monitoring view(= 0.286,P < 0.001)and time efficacy(= 0.178,P < 0.001)were protective factors for the total score of clinical practice behavior.However,gender,whether only child,whether student cadre,average academic achievement and reason of study had no significant influence on the total score of clinical practice behavior(P > 0.05).7.The qualitative results show that the clinical practice experience of the practical nursing students in higher vocational colleges includes: cognitive field: the theoretical knowledge of nursing procedure is gradually weakened,the integrity and standardization of clinical work are insufficient;through clinical practice,theory is closely linked with practice,and the teaching level is uneven.Emotional field: role adaptation is not good,there are differences in professional identity;in the face of some rights and obligations of patients,self-regulation ability is insufficient.Operation skills: skilled in basic nursing operation,lack of specialized nursing skills;enhanced communication ability between nurses and patients;lack of innovation and accuracy in health education.Conclusion1.The time management tendency of vocational nursing students in different periods is at a high level,and the total score tends to decline with the time of practice,in which the highest score of time value is followed by the sense of time efficiency, and finally by the view of time monitoring;the clinical practice behavior of Vocational nursing students is at the upper middle level,and the total score tends to rise first and then decline.2.By introducing the three variables of time value,time monitoring and time efficacy to study the relationship between time management disposition and clinical practice behavior,it is found that time management disposition and clinical practice behavior are positively related in different practice periods,and it can be concluded that time management disposition can positively predict its clinical practice ability.3.Through the regression analysis of the clinical practice behavior of practice nurses in higher vocational colleges,it is found that the factors influencing the clinical practice behavior include the source of students,the level of practice hospital,the value of time,the view of time monitoring and the sense of time efficiency.However,gender,only child or not,student cadre or not,average grade and reason of study have little influence on the total score of clinical practice.4.The real experience of clinical practice of higher vocational nursing students covers the field of cognition,emotion and operation skills.Strengthening the management of interns in hospitals and schools and training their time management skills can improve the clinical practice behavior.
