Objective:To observe the clinical efficacy and safety of Jiawei Buzhong decoction in the treatment of CRF in advanced lung cancer(lung-spleen qi deficiency syndrome of TCM),monitoring the changes of IL-6 in patients before and later treated by the decoction,analyzing the relationship between CRF and the level of IL-6 at the same time.Try to explore the transformation mechanism between CRF and inflammatory microenvironment from the deficiency of Yang qi in TCM.Methods:Patients who cancer-related fatigue in advanced lung cancer(lung-spleen qi deficiency syndrome of TCM),randomly divided into a treatment group and control group(n=36).Treatment group:Under the basic treatment,oral jiawei buzhong decoction(150ml/bag,1 bag/time,3 times/day,provided by Yunnan Province hospital of TCM).Control group:Under the basic treatment,intramuscular injection of thymus polypeptide injection(Specification:1mg;Usage:1mg im qd;Produced by Beijing yanjing pharmaceutical co.LTD).7 days were a course of treatment.All the patients were treated for2 courses.Statistical and discuss the results in the syndrome score of TCM,BFI score,Piper score,KPS scores,IL-6 levels and safety indicators about the 2 groups of patients before treatment,after 1 and 2 course of treatment.Results:The result of the syndrome score of TCM,BFI score,Piper score,KPS score,IL-6 level and security indicators about the 2 groups patients after 1 and 2 course of treatment as follows:1.Comparison of syndrome scores of TCM:In-group comparison:Treatment group:Compared with before treatment,the symptoms of less food,fatigue,shortness of breath,qi deficiency sweating and loose stool were improved after 1 course of treatment(P<0.05),but there was no significant improvement in abdominal distension(P>0.05).After 2 courses of treatment,the above symptoms were much improved(P<0.05).Control group: Compared with those before treatment,after 1course of treatment,fatigue,shortness of breath and qi deficiency sweating were improved(P<0.05).2 course of treatment later,the above symptoms were significantly improved(P<0.05).Comparison between 2 groups after 2 courses of treatment: the treatment group was better than the control group in relieving symptoms of fatigue,loose stool,less food,shortness of breath and qi deficiency sweating(P<0.05).The efficacy of abdominal distension in the 2 groups was comparable(P>0.05).After 2 courses of treatment,the total effective rate was 88.89% in the treatment group and 69.44% in the control group,and the treatment group was better than the control(P<0.05).2.Comparison of Piper score:In-group comparison:Treatment group:Fatigue condition was much improved after 1and 2 course of treatment compared with before treatment(P<0.05).Control group:Compared with before treatment,there was non-significant change in fatigue status after 1course of treatment(P>0.05),and was incrased only after 2 courses of treatment(P<0.05).Efficiency comparison between group:the total effective rate was 86.11% in the treatment group and 66.67% in the control group,and the treatment group was better than the control.3.Comparison of BFI score:In-group comparison:Treatment group,the total BFI score decreased significantly after1 and 2 course of treatment compared with before treatment(P<0.05).In the control group:Compared with before treatment,there was non-significant change in the total BFI score after 1 course of treatment(P>0.05),and was reduced significantly only after 2courses of treatment(P<0.05).Efficiency comparison between 2 groups after 2 courses of treatment:the treatment group was better than control(P<0.05).4.Comparison of KPS score:In-group comparison:Treatment group:Compared with before treatment,the total KPS score increased significantly after 1 and 2 courses of treatment(P<0.05).Control group:Compared with before treatment,there was non-significant change in the KPS score after 1course of treatment,and was increased significantly after 2 course.Efficiency comparison between 2groups:the treatment group was better that the control group(P <0.05).5.Comparison of IL-6 levels:In-group comparison:Treatment group:Compared with before treatment,there was non-significant reduction of IL-6 after 1 course of treatment(P>0.05),and was reduced significantly after 2 courses of treatment(P<0.05).Control group:there was non-significant change in IL-6 no matter when the before,after 1 and 2 courses of treatment(P>0.05).6.Correlation analysis of CRF :The degree of fatigue in patients with advanced lung cancer was strongly significantly correlated with IL-6 level,age,and lung cancer stage.7.Correlation of security indicators:All security indicators were within the normal range before and after treatment,indicating that the 2 groups in this study were safety.Conclusion:1.Jiawei Buzhong decoction can effectively improve the fatigue symptoms of lung cancer patients(lung-spleen qi deficiency syndrome of TCM).2.Jiawei Buzhong decoction can effectively reduce the amount of IL-6 in patients with lung cancer.3.Fatigue in patients with advanced lung cancer may be positively correlated with IL-6 level,that is,the higher the level of IL-6,the more severe the fatigue may be.4.It is possible that the Jiawei Buzhong decoction may reduce the level of IL-6 by improving the inflammatory internal environment to alleviate fatigue.
