Objective: The relationship between the injury degree of KOA and the distribution of plantar pressure to provide a basic theoretical basis for judging the changes of patients with KOA,and to observe the clinical effect of the treatment of unilateral early and mid-stage KOA with "Foot and knee joint treatment".Methods: In the first part,According to the combination of clinical symptoms and radiology Kellgren Lawrence classification,the patients were divided into three groups:KOA early group(29 cases),middle group(32 cases)and late group(27 cases).The related plantar pressure data were collected to study the change of plantar pressure of unilateral KOA.in the second part,Then the clinical efficacy of "Foot and knee treatment" was collected Patients(part of the patients were from the correlation study of the change of sole pressure of unilateral KOA)were randomly divided into the control group(29 cases)and the observation group(28 cases);Both groups were treated with basic treatment(intra-articular injection of sodium hyaluronate),the observation group combined with functional exercise and traditional Chinese medicine fumigation,and compared the VAS score,excellent rate,JOA score and the change of sole pressure of the two groups.Results:(1)In the study of the correlation between the changes of plantar pressure in unilateral KOA,the proportion of healthy side pressure and the amount of pelvic rotation gradually increased with the development of KOA.(2)In the study of the correlation between the changes of plantar pressure in unilateral KOA,the distribution of plantar pressure in the affected side of the patient's plantar dynamic "characteristic footsteps" gradually distributed to the outside,and the proportion of T1,t2-5 pressure decreased;among them,the distribution of M5,MF,LH pressure increased with the development of KOA,and M5,LH can be used as an independent factor in the clinical stage of KOA;T1,T25 and the degree of KOA injury were negatively correlated,t2-5 can be used as an independent factor It is an independent influencing factor of clinical stage of KOA.(3)In the study of the correlation between the changes of sole pressure in unilateral KOA,the pressure distribution in front of the patient's dynamic "characteristic footsteps" on the healthy side tended to be laterally distributed;the pressure distribution of M2,m3 and M4 gradually increased,among which m3 had a significant correlation and could be used as an independent influencing factor of clinical stages of KOA.(4)In the observation of the clinical effect of the treatment of unilateral early and mid-term KOA with "foot and knee treatment",57 patients completed the clinical observation.In terms of VAS score,there was no significant difference between the two groups in the VAS score at the second week of treatment.By t-test,P = 0.370(P > 0.05),and there was no significant difference between the two groups at the fourth,sixth and eighth weeks of treatment.By t-test,P value was less than 0.05,and the difference was statistically significant.(5)In the observation of the clinical effect of the treatment of unilateral early and mid-term KOA with the method of "treating foot and knee together",the improvement of VAS score in the eighth week was compared.In the observation group,9 cases were excellent,19 cases were good,1 case was fair,0 case was poor,the excellent and good rate was 68.96%;in the control group,2 cases were excellent,21 cases were good,5cases were fair,0 case was poor,the excellent and good rate was 68.96%,P = 0.018(P <0.05),the difference was statistically significant.(6)In the observation of clinical effect of "foot and knee joint treatment" in the treatment of unilateral early and mid-term KOA,there was no significant difference between the two groups in the treatment of JOA knee osteoarthritis at the 2nd and 4th week,P > 0.05 by t-test,and there was no significant difference in the treatment of JOA knee osteoarthritis at the 6th and 8th week,by t-test P value was less than 0.05,the difference was statistically significant.(7)In the clinical observation of the treatment of unilateral early and middle term KOA with the method of "foot and knee treatment",the static plantar pressure distribution and pelvic rotation of the two groups after the 8th week of treatment were compared.Through t-test,P value was less than 0.05,the difference was statistically significant.(8)In the clinical observation of the treatment of unilateral early and mid-term KOA with "foot and knee treatment",the proportion of t2-5 and MF pressure in the dynamic "characteristic foot" plantar pressure distribution of the affected side in the two groups after the 8th week of treatment was tested by t test,P value was greater than 0.05,the difference was not statistically significant;the proportion of T1,M5 and LH pressure was tested by t test,P value was less than 0.05,the difference was statistically significant.(9)In the clinical observation of the treatment of unilateral early and middle term KOA with "foot and knee treatment",the proportion of M4 pressure on the healthy side in the dynamic "characteristic foot" plantar pressure distribution after the 8th week of treatment in the two groups was t-test,P = 0.830(P > 0.05),indicating that there was no statistical significance between the groups;the proportion of M2 and M3 pressure was t-test,P value was less than 0.05,the difference was statistically significant.Conclusion: For the patients with unilateral early and middle term KOA,the two methods have achieved a certain effect,but the "foot and knee treatment" method has a better effect in relieving pain,improving swelling,joint activity and foot pressure,which is worthy of clinical promotion.
