Objective: To observe the efficacy and safety of electroacupuncture in the treatment of chemotherapy-induced peripheral neuropathy in multiple myeloma,and to detect the expression level of mi RNA-21 in the serum of patients admitted to the group,and to explore the therapeutic effect of electro-acupuncture in the treatment of peripheral myeloma associated with chemotherapy in multiple myeloma.Possible mechanism.Methods: Patients with multiple myeloma who were admitted to our hospital's hematology department from June 2018 to February 2020 were selected.Western medicine meets the diagnostic criteria for multiple myeloma with CIPN,and Chinese medicine meets the scope of syndrome and paralysis.The patients were randomly divided into electroacupuncture group and control group by random number method,with 30 cases in each group.The control group was treated with the original chemotherapy regimen(mainly with bortezomib-containing chemotherapy regimen)and took gabapentin capsules.The electro-acupuncture group added electro-acupuncture to the control group's treatment regimen,25 min / time,1 time / day,6 Times /courses,a total of 2 courses,with an interval of 1 day between the twocourses.Before and after treatment,patients in both groups were evaluated with questionnaires using the FACT / GOG-Ntx neurotoxicity rating scale,the clinical version of the total neuropathy score scale(TNSc),and the NRS pain score scale.Including motor nerve conduction velocity and sensory nerve conduction velocity),serum mi RNA-21 expression level,blood routine,liver and kidney function and other related indicators,and record adverse reactions during treatment.Results:A total of 2 cases were lost during the study,one in each of the electroacupuncture group and the treatment group,and a total of 58 patients were included in the electroacupuncture group and 29 in the control group.1.The total effective rates of TCM symptoms in electroacupuncture group and treatment group were 86.2% and 65.55%,respectively.Electroacupuncture group was better than control group.The total scores of TCM symptoms after treatment in both groups were lower than before treatment(P <0.05).The improvement of TCM symptoms in electroacupuncture group was better than that in control group(P <0.05).2.The FACT / GOG-Ntx scale score,the clinical version of the total neuropathy score scale(TNSc),and the NRS pain score of the two groups of patients decreased after treatment(P <0.05),and the nerve conduction velocity(NCV)increased after treatment.(P <0.05).Comparing the groups,the effect of improving the peripheral neuropathy index of theelectroacupuncture group was better than that of the control group(all P<0.05).3.The levels of mi RNA-21 in serum decreased before and after treatment in the two groups of patients(P <0.05).Comparing between groups,the mi RNA-21 level in the electroacupuncture group was better than that in the control group(P <0.05).4.The hemoglobin level increased after treatment in the two groups(P <0.05),and there was a statistically significant difference between the groups(P<0.05).After treatment,the levels of leukocytes and creatinine in the two groups decreased(P <0.05),and there was no significant difference between the groups(P> 0.05).There were no significant differences in platelet,alanine aminotransferase,serum potassium,and serum calcium levels between the two groups after treatment(P <0.05).5.During the treatment,no adverse reactions such as local persistent redness,swelling and pain,local skin itching,nausea and vomiting,and dizziness and headache were observed.Conclusion :1.Electroacupuncture therapy for multiple myeloma chemotherapy-induced peripheral neuropathy can improve CIPN symptoms,effectively improve the patient's FACT / GOG-Ntx score,NRS pain score,TNSc score,and nerve conduction speed.Peripheral neuropathy has significant therapeutic effects.2.Electroacupuncture can reduce the expression level of mi RNA-21 inmultiple myeloma serum.The mechanism may be related to the regulation of JAK2 / STAT3 signaling pathway and mi RNA-21.3.Electroacupuncture can obviously improve the symptoms of TCM such as numbness of the limbs,pain in the extremities,fatigue and fatigue,and loss of sensation.4.Electroacupuncture has no obvious adverse reactions and good safety,which can improve the anemia performance of multiple myeloma to a certain extent.
