| Objective:Multiple myeloma is a malignant proliferative disease of plasma cells with unknown etiology.With the continuous improvement of MM diagnosis and treatment,the incidence of chemotherapy-induced peripheral neuropathy is also gradually increasing.The comprehensive therapy of traditional Chinese medicine has a considerable effect on the treatment of multiple myeloma chemotherapy-associated peripheral neuropathy.However,a complete syndrome differentiation and treatment system has not yet been formed in the study of TCM syndrome types of MM-CIPN.Through statistical analysis of MM-CIPN,this study explored the distribution and analysis of TCM syndromes of MM-CIPN,in order to lay a foundation for the development of standardized diagnostic criteria of TCM syndromes of MM-CIPN.Method:In this retrospective study,patients with MM who were admitted to the Hematology Department of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from December 2016 to December 2020 and who met the diagnostic criteria of MM-CIPH were selected.By referring to relevant books and literatures,the registration form of MM-CIPN information investigation on the four diagnoses of traditional Chinese medicine was developed,and unified information collection related to TCM symptoms of patients was carried out.Excel was used to establish a database,and the data was imported into SPSS 25.0 software for descriptive frequency frequency analysis and cluster analysis.Results:(1)General information:Medical records of 159 patients who met the inclusion criteria were screened out,including 86 males and 73 females,with a minimum age of 34 years and an average age of 61.65 years.The peak age of onset was 61-70 years old.(2)TCM symptoms:limb numbness,limb tingling,fear of cold limbs,fatigue,poor flexion and extension,thin white moss,abdominal distension and discomfort.stool stool,insomnia,waist and knee sour and soft,less qi lazy speech,chest tightness with phlegm and other symptoms appear more frequently.(3)Classification of TCM syndrom types:According to the results of cluster analysis and combined with clinical experience,TCM syndrom types of MM-CIPN are divided into the following four types:qi deficiency and blood stasis syndrome,spleen-kidney Yang deficiency syndrome,liver-kidney Yin deficiency syndrome,phlegm and blood stasis obstruction syndrome.(4)The TCM syndrome types of 159 patients were included:according to the four TCM syndrome types obtained,the 159 patients were differentiated into different types,and the results showed that 68 cases(42.77%)had qi deficiency and blood stasis syndrome,34 cases(21.38%)had spleen-kidney Yang deficiency syndrome,29 cases(18.24%)had phlegm and blood stasis syndrome,and 28 cases(17.61%)had liver-kidney Yin deficiency syndrome.The distribution of TCM syndromes in patients with different genders was basically the same,and the distribution was very close to the overall distribution.The chi-square test showed no significant difference in the gender of MM-CIPN patients with the four syndromes(P>0.5),suggesting that there was no correlation between the classification of TCM syndromes and the gender of patients.(5)The distribution of TCM syndromes in different age ranges:in 34-40 years old,the proportion of phlegm and blood stasis syndromes is the highest,75%;In 41-50 years old,the proportion of phlegm and blood stasis syndrome was the highest,accounting for 44.5%;Among 51-60 years old,the proportion of Qi,blood and blood stasis syndrome was the highest,accounting for 56.8%;Among patients aged 61-70 years,the proportion of Qi deficiency and blood stasis syndrome was the highest,accounting for 43.6%;In 71-80 years old,the proportion of spleen-kidney Yang deficiency syndrome was the highest,accounting for 46.2%.Conclusion:TCM syndrome types of MM-CIPN can be summarized into the following four types:Qi deficiency and blood stasis syndrome,spleen and kidney Yang deficiency syndrome,liver and kidney Yin deficiency syndrome,phlegm and blood stasis obstruction syndrome.This disease is located in the meridians of qi and blood,and is closely related to the spleen,kidney and liver.It is usually manifested as deficiency of qi and Yang in the spleen and kidney,deficiency of Yin in the liver and kidney,and blood stasis runs through the whole disease process. |
