Survey On The Status Quo Of The Construction Of General Medical Disciplines In General Hospitals Of Third Level In Henan Province

Research purpose: To understand the current status of general medical discipline construction in 28 tertiary general hospitals in Henan Province through a questionnaire survey.The research content includes the construction of generalized training bases for general practitioners,the investigation of the ability of subject leaders,the professional recognition of clinical teachers in general medical departments Education and training status and basic service capabilities,general outpatients' understanding of general practice and general practice needs;analysis of the above questionnaire results,elaboration of the current status of discipline construction,summarizing the effectiveness of general hospital medical department construction in general hospitals,and discovering the process of discipline construction Existing problems and difficulties;combined with the actual development of general practice in Henan Province,put forward suggestions to the health department to promote the sustainable development of general medicine in the three-level general hospitals in our province.Research methods: The literature analysis method,thematic group discussion method,expert consultation method and questionnaire survey method were used to investigate and investigate the construction of the general medical discipline in the third-level general hospitals in Henan Province.First,the questionnaire survey on the construction of general medicine disciplines in the third-level general hospitals in Henan Province was designed through the literature analysis method,thematic panel discussion method,and the expert consultation method.A general descriptive analysis was conducted,and age,work,and time spent in teaching were expressed by composition ratio and mean ± standard deviation.Research results:(1)General medicine is recognized as a well-known academic discipline in European and American countries with a hundred years of history.The reputation and competitiveness of the discipline are constantly increasing,and its discipline construction system and system are relatively mature.European and American national hospitals have no grades,and the research of general medicine construction focuses on primary care clinical research,general practitioner teaching experience,general practitioner practice model research,general resident training model,and community epidemiology research.Researches on the construction of general sciences in Beijing,Shanghai,Zhejiang and other places in China are concentrated in the fields of base construction,teacher construction,outpatient clinic construction,and resident management,but there are problems such as a single research object and insufficient research content.(2)Through a questionnaire survey of 28 general practice base directors,the surveyed hospitals have set up general practice wards and outpatient clinics independently,and are affiliated with grass-roots practice teaching bases.The basic teaching equipment of the ward is well-equipped,but most of the general outpatient clinics at the base have not met the national requirements.The average weekly outpatient time of the general director is(2.19 ± 1.07)days,the average weekly outpatient teaching time(2.03 ± 1.22)days,and the average weekly administrative time(3.22 ± 1.73)days.88.89% of the general medicine departments established general education departments and training leadership groups.83.33% of hospitals have established management methods for training bases.94.44% of the hospitals have a base management director responsible system and full-time / part-time secretary.However,in the field of scientific research,only 35.7% undertook the subject of general practice.(3)Through a questionnaire survey of 268 clinical teachers in general practice bases,the majors before the practice are concentrated in internal medicine(75.0%),and the form of teacher certification is mainly provincial and above general training courses(55.0%).62.3% of general practitioners believe that the work pressure is very high,and the main source is the heavy workload.The content of the general teacher training focuses on medical knowledge(68.3%),professional literacy(63.4%),ability to communicate with patients(58.9%),preventive health care(36.2%),clinical operation skills(22.8%),public health services(17.9 %),Health education(13.4%).The general medical education content is concentrated on clinical medicine(62.3%),preventive medicine(54.1%),rehabilitation medicine(13.4%),health education(32.5%),emergency and first aid(33.2%),mental health(19.7%),Nutrition(29.1%),medical imaging(29.1%),communication and communication(22.8%),management(7.8%),family planning technical guidance(4.1%).(4)Through a questionnaire survey of 840 general outpatients,74.8% of the patients had more than 3 visits,the purpose of which was to prescribe medicine and issue checklists(28.3%),health checkups(22.1%),and follow-up visits for chronic diseases(20.5%).21.1% of the patients believed that the general practitioner was Dr.Wan Jinyou,and only 15.5% of the patients had knowledge of the community's first referral system and two-way referral system.Among the general service models that patients hope to receive,GP visits(60.1%),telephone consultation(58.8%),and outpatient medical treatment(56.2%).83.6% have signed a family doctor,90.4% of patients hope that their family doctor can regularly carry out health education,disease science,psychological counseling,and 88.9% of patients hope that their family doctor can regularly follow up and adjust drugs for patients with chronic diseases.Outpatients believe that the qualities that general practitioners should possess focus on excellent medical technology(95.6%),comprehensive medical knowledge(98.4%),high sense of responsibility,love,compassion(96.4%),good expression skills and Communication skills(87.9%).Research conclusion: Through surveys of the directors of the general practice bases of 28 tertiary general hospitals in Henan Province,268 clinical teachers of the general practice bases,and 840 general outpatients,it is found that the teaching resources of the general practice bases are reasonably allocated,and the rules and regulations are relatively perfect,A high sense of professional achievement and identification of all teachers.However,scientific research is a short board of disciplines,with low quality and few results.The general teacher training system is imperfect,there is a lack of teacher certification standards,and the general medical services are not perfect.Residents lack awareness of GPs and their policies to benefit people.Therefore,it is necessary to standardize the construction of bases,strengthen the construction of scientific research platforms,establish a teacher training system,and at the same time strengthen the promotion of the concept of the whole subject.
