ObjectivesThe purpose of this study is to investigate the current situation of health quotient of patients with coronary heart disease after PCI,to analyze the influencing factors of health quotient,treatment compliance and quality of life and the relationship among them,and to explore the effect of nursing care based on action research in patients with coronary heart disease after PCI,so as to provide new reference for clinical nursing.Methods(1)Using the convenient sampling method,from September 6,2018 to January 10,2019,288 patients with coronary heart disease who met the criteria after PCI in the Department of Cardiology of a third-class hospitals in Hengyang City were investigated with patient general condition questionnaire,brief health quotient index 20-item questionnaire,treatment compliance questionnaire after PCI and Chinese cardiovascular patients' quality of life assessment questionnaire.To understand the current situation of health quotient,treatment compliance and quality of life of patients with coronary heart disease after PCI,to find out the influencing factors,and to explore the relationship among them.272 valid questionnaires were collected,with an effective rate of 94.4%.(2)Patients with coronary heart disease after PCI in the Department of Cardiology of a third-class hospital in Hengyang City were selected as intervention subjects,with 42 patients in intracardiac area 2 as intervention group and 42 patients in intracardiac area 1 as control group.The general data,health quotient score,treatment compliance score,quality of life score and low density lipoprotein cholesterol(LDL-C)were compared between the two groups.The patients in the control group were given routine nursing care in the department of cardiology,and the intervention group was given nursing care based on action research and routine nursing for a period of nine months.The scores of health quotient,treatment compliance,quality of life and LDL-C were compared between the two groups.In the course of the study,two patients in both groups dropped out,and the final subjects in both groups were 40 patients with coronary heart disease after PCI.(3)SPSS 23.0 software was used for statistical analysis,and descriptive statistics,chi-square test,t-test,pearson correlation analysis,one-way ANOVA,multiple linear regression analysis and other statistical methods were used to analyze.Results(1)The total average score of health quotient of patients with coronary heart disease was(57.54±10.47),the score range was from 31 to 87,the average score of self-care was(11.4±3.69),the average score of health knowledge was(2.94±2.69),the average score of mental health was(16.69±3.00),the average score of life style was(21.24±4.96),and the average score of life skill was(5.59±2.11).Age,education,residence and drinking habit are the main influencing factors of health quotient,which explains 39.4% of the total variation of the equation.(2)In the treatment compliance of patients with coronary heart disease,44.1% of the patients had good drug compliance,55.9% of the patients had poor drug compliance,78.3% of the patients had not reexamined coronary angiography,and 80.1% of the patients had not reached the standard of exercise.Age,education,residence and drinking habit were the main influencing factors of treatment compliance,which explained 29.3% of the total variation of the regression equation.(3)The average score of quality of life in patients with coronary heart disease was(66.01±13.48),and the score range was from 32 to 95.The main influencing factors were gender,the number of stents and drinking habit,which explained the variation of the regression equation by 15.6%.(4)Health quotient of patients with coronary heart disease after PCI was positively correlated with treatment compliance and quality of life;all dimensions of health quotient were positively correlated with treatment compliance except mental health and lifestyle compliance;except mental health and lifestyle compliance,all dimensions of health quotient were positively correlated with quality of life;except lifestyle compliance,all dimensions of treatment compliance were positively correlated with quality of life.(5)Before intervention,there was no significant difference in general data,health quotient,treatment compliance,total score of quality of life and LDL-C,between the intervention group and the control group after PCI(P>0.05).(6)After intervention,the scores of health quotient and all dimensions of patients with coronary heart disease after PCI in the intervention group were significantly higher than those before intervention(P<0.05),and the scores of health quotient and all dimensions except life style in the intervention group were significantly higher than those in the control group except life style(P<0.05).(7)After intervention,the scores of treatment compliance and all dimensions of patients with coronary heart disease after PCI in the intervention group were significantly higher than those before intervention(P<0.05),and the scores of all dimensions of treatment compliance in the intervention group after PCI were significantly higher than those in the control group(P<0.05).(8)After intervention,the score of quality of life of patients with coronary heart disease after PCI was significantly higher than that before intervention,and the score of quality of life of patients with coronary heart disease after PCI after intervention was significantly higher than that of the control group(P<0.05).(9)After intervention,LDL-C in patients with coronary heart disease after PCI was significantly lower than that before intervention,and after intervention,LDL-C in patients with coronary heart disease after PCI was significantly lower than that in the control group(P<0.05).Conclusions(1)The health quotient,treatment compliance and quality of life of patients with coronary heart disease after PCI were at a low level.(2)There was a positive correlation among health quotient,treatment compliance and quality of life in patients with coronary heart disease after PCI.(3)Nursing care based on action research can improve the health quotient,treatment compliance and quality of life of patients with coronary heart disease after PCI,reduce the level of LDL-C,and provide value for guiding patients with coronary heart disease after PCI in the future.
