Helicobacter pylori is a type of carcinogen that causes gastric cancer and can cause a variety of gastrointestinal diseases,such as chronic gastritis,peptic ulcer,reflux esophagitis,etc.Eradication of Hp is one of the important ways of treatment for a variety of gastrointestinal diseases.As the phenomenon of antibiotic resistance is becoming more and more obvious,the eradication rate of Hp is also decreasing.Some patients cannot eradicate Hp even after initial treatment or even multiple treatments,and repeated treatments will lead to aggravation of the drug resistance problem,making remedial treatment of Hp more difficult.This clinical observation aims to provide a theoretical basis for the combined treatment of Hp infection with traditional Chinese medicine qilian decoction combined with tetrad therapy based on furazolidone to remedy Hp infection(syndrome of dampness and heat in spleen and stomach),as well as a new idea and method for the treatment of Hp infection.Part one:theoretical researchFrom the perspective of modem medicine on Hp infection of epidemiology,pathogenesis,diagnostic methods and treatment status at home and abroad were reviewed analysis,concluded that modern medicine on etiology of Hp infection,pathogenic mechanism and clinical research has obtained certain achievements,but as the phenomenon of antibiotic resistance,simple to use western medicine remedial treatment of Hp infection eradication rate is declining,and there exists large side effects,patient compliance of weakness.Then,traditional Chinese medicine has a certain advantage in remedial treatment of Hp infection,from medicine to see Hp infection belongs to the category of "pathogenic qi" of traditional Chinese medicine," pattern of healthy qi deficiency and lingering pathogen " is the basic pathogenesis of Hp infection in delay no more,spleen weakness is the basis of the disease,damp and hot is the pathological factors of Hp infection and disease,syndrome differentiation and treatment is an effective method of remedial treatment,basic treatment for the spleen and replenishing qi,sinochem wet.In addition,based on the theoretical analysis of the compatibility of qilian decoction and the modern pharmacological research,the theoretical mechanism of the therapeutic effect of qilian decoction on the remedial treatment of Hp infection was expounded.Part two:clinical researchObjective:Through clinical grouping trial,objectively evaluate the safety and efficacy of Qilian decoction combined with furazolidone-based quadruple therapy in clinical Observation of remedial treatment of Hp positive spleen deficiency and heat syndrome.Methods:A total of 63 patients who met the inclusion criteria were included in this study.They were divided into a treatment group and a control group,32 in the control group,and 31 in the treatment group.The treatment group was treated with furazolidone-based quadruple therapy with Qilian decoction for 14 days,The control group was treated with furazolidone-based quadruple therapy only for 14 days.The clinical symptoms before and after treatment were observed in two groups of patients,and the symptom score was calculated before and after treatment.The eradication rate was measured 4 weeks after the course of foot treatment,and the adverse reactions during the treatment were recorded.Result:(1)The eradication rate of Hp in the two groups was 93.33%in the treatment group,80.00%in the control group,and P>0.05.Although the eradication rate in the treatment group was higher than the control group,it was not statistically significant(2)After the treatment,the individual symptoms were compared by rank sum test.The results showed that the main symptoms were epigastric fullness,epigastric pain,bad odor in the mouth,and secondary symptoms of thoracic acid,hypo appetite,and nausea and vomiting.P<0.05,the difference was statistically significant.The significance of the study indicates that the treatment group is better than the control group in improving such symptoms of TCM;the two groups have improved P>0.05 in improving symptoms of dry mouth,bitterness and yellow urine,and the difference is not statistically significant.There are differences in bitterness and yellow urine.(3)A total of 5 adverse events occurred in this trial.There were 2 cases in the treatment group,1 patient had diarrhea,and 1 patient had headache.The control group consisted of 3 cases,including 2 cases of diarrhea and 1 case of mild increase in liver function.After rank sum test,P>0.05,the difference was not statistically significant.Due to the mild symptoms,the above patients did not meet the shedding standard and did not affect the test results.Conclusion:Qilian decoction combined with furazolidone quadruple therapy has significant effect on symptom score,syndrome score,H.pylori eradication status and effective rate before and after treatment with fewer adverse reactions.verification.The current research shows that Qilian Decoction combined with furazolidone quadruple therapy has achieved preliminary clinical efficacy in treating spleen deficiency damp-heat type Hp infection with good safety and is worthy of further clinical research.
