BackgroundVestibular Migraine(VM),also known as migraine vertigo,is considered a variant of migraine and its main clinical manifestations are vertigo,dizziness,or balance disorders related to migraine.Although VM has the second highest incidence of vertigo,the diagnosis rate is only 4%-10%,indicating that the clinical misdiagnose and missed diagnosis rate of VM is high.Although the diagnosis of VM mainly depends on the medical history and clinical manifestations,related auxiliary examinations are also essential.Videonystagmography(VNG)can record in detail the eye movement trajectory during examination and find faint nystagmus to evaluate patients with suspected vestibular dysfunction.Therefore,this study uses VNG to examine VM patients and healthy people,and compares the performance of two groups of VNG to determine its diagnostic value in VM.Objective1.To compare the VNG performance of VM patients and healthy people.2.To explore the diagnostic value of VNG inspection in VM.MethodsA total of 100 subjects were selected,including 55 confirmed VM patients and 45 matched healthy subjects.Inclusion criteria:Diagnosis according to the diagnostic criteria for VM in the diagnostic criteria for the third edition of the International Headache Classification(ICHD-Ⅲ)in 2018;Healthy Subject Group:Normal physical development,no obvious difference in race,age,blood pressure,gender,education level and VM group,and never experienced vertigo or migraine.All subjects were examined for VNG after detailed medical history was collected and contraindications were excluded.Examination content:1.Visual eye movement system check:saccade test,stable tracking test,optokinetic test;2.Vestibular eye movement system examination: Caloric test,head shaking nystagmus(HSN);3.Spontaneous nystagmus;4.Induced nystagmus examination: static positional test.Calculate the abnormal rate of VNG performance in each experiment of VM group and healthy subject group and use χ2 test to compare between groups.Results1.There was no significant difference in gender,age and education between the two groups(P> 0.05).2.Compared with the healthy subject group,the abnormal rate of VNG in the visual eye movement system examination,vestibular eye movement system examination,and positional test was higher in the VM group,and the differences were statistically significant.(P <0.05).3.The abnormal rate of VNG in spontaneous nystagmus in the VM group was higher than that in the healthy subjects,but the difference between the two groups was not statistically significant(P> 0.05).Conclusions1.VNG examination is helpful for VM diagnosis.2.The manifestations of VNG in patients with VM suggest signs of damage to the center and periphery.
