Objective:To observe the clinical effect of acupuncture combined with traditional Chinese medicine on patients with hyperactivity of liver-yang syndromes vestibular migraine.Materials and methods: Patients with Vestibular Migraine(VM),hyperactivity of liver-yang syndromes,hospitalized in the Department of Otorhinolaryngology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine between April 2021 and June 2022 was selected,and randomly divided into control group and experimental group,with 30 cases in each group.The control group was given flunarizine hydrochloride capsule10 mg once a day orally for 14 days.Vestibular rehabilitation(VR)was completed once a day for three months.On the basis of control group,experimental group was given addition and reduction of Tianmagouteng Decoction plus or minus 1 dose a day three times a day for 14 days.Acupuncture at Yintang,Baihui,and bilateral Taiyang,Shuaigu was performed once a day for 14 days.Video head impulse test and Transcranial color Doppler ultrasound(TCD)were performed at the time of before treatment,4 weeks and 12 weeks of treatment,respectively,and the Traditional Chinese Medicine Syndrome Score Scale(TCMSS),headache score scale and Dizziness Handicap Inventory(DHI),objective test data and the scales at each stage of the treatment are summarized,and the curative effect is assessed and analyzed statistically.Result:1.There was no significant difference in baseline between the experimental group and the control group at the time of enrollment(P > 0.05).2.vHIT gain in the vertical semicircular canal of the two groups was decreased at all stages(P< 0.05),and vHIT gain in the experimental group was lower than that in the control group at all stages(P < 0.001).3.The abnormal rate of TCD in each stage of the two groups was improved compared with the previous stage(P < 0.05).At 4 weeks,the experimental group was superior to the control group(P=0.013).After 12 weeks of treatment,the TCD results of all patients in the experimental group were better than that of the control group(P=0.005).4.The total score of DHI in both groups was significantly improved at all stages(P < 0.001),and the experimental group was better than the control group at all stages(P < 0.05).5.Headache score scale in both groups were improved at all stages(P < 0.001),the test group was superior to the control group at 4 weeks of treatment(P=0.034),and the test group was superior to the control group at 12 weeks of treatment,with a statistically significant difference(P < 0.001).6.TCMSS in both groups was significantly improved at all stages of treatment(P < 0.001),and the experimental group was better than the control group(P < 0.05).7.After 4 weeks of treatment,the effective rate of the experimental group was 96.67%,which was better than that of the control group(83.34%,P=0.007).At 12 weeks,the efficacy of both groups was improved(P < 0.001),and the experimental group was better than the control group(P=0.020).Conclusion:1.Acupuncture,TCM combined with flunarizine hydrochloride can improve vertigo,headache and TCM syndrome and other symptoms,and both short-term and long-term efficacy are better than oral flunarizine hydrochloride alone.2.Oral flunarizine hydrochloride combined with acupuncture and TCM can reduce the vertical semicircular canal gain value of video head impulse test in patients with vestibular migraine,improve cerebral vascular tension and hemodynamics,and the combined effect of traditional Chinese and western medicine is more excellent.3.As a central vestibular disease whose pathogenesis is related to a variety of targets and signaling pathways,the overall efficacy of acupuncture and TCM combined with flunarizine hydrochloride is superior to flunarizine hydrochloride,which may be related to acupuncture and TCM simultaneous regulation of multi-target and multi-pathway by addition and reduction of Tianmagouteng Decoction.
