Objective:The objective of this study was to distinguish Vestibular migraine(VM)from Meniere's disease(MD)by comparing age,sex,family history,audiological and vestibular test results in patients with both.To provide a better diagnosis and treatment plan for suspected patients with vestibular migraine,we compared the age,gender,family history,hearing function and vestibular function of suspected patients with vestibular migraine and vestibular migraine.Methods:60 Vestibular migraine patients,60 Meniere's patients and 46 suspected patients with vestibular migraine were studied.All patients were asked for detailed medical history and examined by otoscopy,bithermal caloric test,video head impulse(vHIT)and audiological test,some patients were tested with vestibular myogenic evoked potentials(VEMP)in acute or subacute phase.The listening function and vestibular function of VM and suspected patients with VM,VM and MD were analyzed.Results:(1)There were 46 patients with suspected vestibular migraines,including 8males and 38 females,and sex ratio is1:4.75,the age of the patients range from 18 to 71 and the average age is(50.87±13.52).Family history of these patients were80.43%(37/46).Sensorineural hearing loss was present in 60.87(28/46).50%(23/46)of the them had abnormal bithermal caloric test findings,86.96%(40/46)abnormal vHIT.24 patients were tested with VEMP,37.5%(9/24)abnormal cVEMP,79.17%(19/24)abnormal oVEMP.(2)Among 60 VM patients,including 6 males and 54 females,and sex ratio is 1:9,the age of the patients range from 8 to 74 and the averageage is(51.00±10.85).Family history of these patients were66.67%(40/60).Sensorineural hearing loss was present in 63.33%(38/60)of VM patients.31.67%(19/60)of the them had abnormal bithermal caloric test findings,88.33%(53/60)abnormal vHIT.26 patients were tested with VEMP,50%(13/26)abnormal cVEMP,88.46%(23/26)abnormal oVEMP.(3)Among 60 MD patients,including 30 males and 30 females,and sex ratio is basically the same,the age of the patients range from 24 to 76 and the average age is(55.90±13.20).Family history of these patients were 6.67%(4/60).None of the MD patients had Normal hearing.78.33%(47/60)of the them had abnormal bithermal caloric test findings,50%(30/60)abnormal vHIT.17 patients were tested with VEMP,58.82%(10/17)abnormal cVEMP,47.06%(8/17)abnormal oVEMP.(4)The difference of audiological test,bithermal caloric test,vHIT,oVEMP between VM and MD was statistically significant,and the difference of cVEMP abnormal rate was not statistically significant(P<0.05).The difference of audiological test,bithermal caloric test,vHIT,oVEMP between VM and suspected patients with VM was not statistically significant(P>0.05).Conclusions:(1)In VM patients,there are more females than males,and sex ratio of the MD patients is basically the same.Compared with MD patients,there is an obvious family history of VM patients.(2)Both VM and MD patients can have semicircular canal and otolith organ function abnormality.vHIT and oVEMP have higher abnormality rate in VM patients,and the bithermal caloric test has higher abnormality rate in MD patients.(3)Both the VM and the MD have different levels of hearing loss.The hearing curve of VM is various and the hearing loss of high frequency is more.All patients with Meniere's disease had low frequency hearing loss.(4)Some patients do not meet the diagnostic criteria of the VM,but the clinical features are similar to VM,and the results are not different from the VM,which can be experimental treated according to the VM.
