Objective:To summarize characteristics of vestibular function in young and middle-aged female patients with vestibular migraine from high area and provide more evidence for diagnosis and differential diagnosis.Moreover,to help to discuss the possible mechanism by the Results of vestibular function.Contents and methods: 40 VM patients screened by Nasal-row criteria were collected as the case group,and 40 young and middle-aged healthy female volunteers of the same age were collected as the control group.The clinical manifestations of the patient history were recorded in detail in the case group;The mean number of Slow phase velocity(SPV)of spontaneous nystagmus in two groups was calculated;The abnormal rate of oculomotor tests,positional test,caloric test and VHIT in each group were calculated and compared between the two groups.The abnormal rate of vestibular function examination in patients with disease duration of > for 5 years and disease duration of <5 years in the case group was calculated,and the two groups were compared.Statistical treatment was performed,and P<0.05 was considered to be statistically significantResults:In the case group,27.5%(11)of the patients' disease course is more than 5 years and 72.5%(29)of the patients' disease course is less than 5 years.The most common form of vertigo is spontaneous vertigo which is 75% and the most common cause is sleep disorders(29.6%)and the most common accompanying symptoms are photophobia and fear of sound.In VM group,the spontaneous nystagmus mean is 4.83±1.472°/s.In control group,it is 1.75±0.354°/s.Comparing two groups,P<0.001 and the differences has statistical significance.In gaze nystagmus and optokinetic nystagmus,there is no abnormality.The abnormal rate of smooth pursuit is 5% in VM group and 0 in control group.Comparing the two groups,P<0.05 and it has statistical significance.In position test of VM group,the nystagmus mean is 4.96±0.970 and the abnormal rate is 35%.In control group,it is 5%.Comparing two groups,P<0.001 and it has statistical significance?In caloric test,the abnormal rate is 30% in VM group and 2.5% in control group.Comparing the two groups,P<0.001 and it has statistical significance.In VM group,about abnormal vestibular function,the patients who have more than 5 years disease course are 81% and the patients who have less than 5 years disease course are 55.1%.The comparison between two groups has statistical significance.Conclusion: In VM patients,The most common symptom is spontaneous vertigo.Fear of light and sound is an accompanying symptom with relatively high specificity.The vestibular function examination during VM acute attack could find central and peripheral vestibular dysfunction.The most common one is persistent positional nystagmus which shows in a form of low speed,persistent,no incubation period,no fatigue.Other successive forms are spontaneous nystagmus,cold and hot gas stimulation,VHIT and abnormal eye movement.The longer the disease course lasts,the more common vestibular function is injured.For the young and middle-aged females who get recurrent paroxysmal vertigo,position test will find persistent and low-speed nystagmus accompanying with headache,fear of light and sound which strongly suggest the possibility.
