Objective:Through the study of TCM Syndromes of liver metastasis of colorectal cancer,the purpose of this project is to provide reference for TCM clinical syndrome differentiation and treatment of liver metastasis of colorectal cancer,to better play the role of TCM in advanced colorectal cancer,to control the progress of disease of liver metastasis of colorectal cancer through TCM precise syndrome differentiation and treatment combined with other disciplines,to improve the quality of life,and to increase the probability of clinical cure In order to achieve the long-term survival of patients.Methods:In this study,335 cases of colorectal cancer with liver metastasis were collected for clinical basic data and four diagnostic information of traditional Chinese medicine.Frequency analysis,cluster analysis,and other multiple statistical algorithms were used to explore the distribution characteristics of TCM syndrome types of colorectal cancer with liver metastasis,and the related factors were studied.The purpose of this study is to provide reference for TCM clinical syndrome differentiation and treatment of colorectal cancer with liver metastasis,so as to better play the role of TCM in the treatment of colorectal cancer with liver metastasis Role of advanced colorectal cancer.Results:(1)In this study,335 patients with liver metastasis from colorectal cancer were included,including 193 males,accounting for 57.61% of the total number,144 females,accounting for 42.99% of the total number.Among 335 subjects,the proportion of patients younger than 30 years old was the least(1.49%),and the proportion of patients aged 61-70 years old was the most(25.97%).In the past,56 patients had bad habits such as smoking and drinking,accounting for 16.72%;the patients without smoking and drinking history accounted for a large proportion,accounting for 83.28%.There are 81 patients with other combined diseases,18 patients with history of coronary heart disease,accounting for5.35%;8 patients with digestive tract disease,accounting for 2.39%;30 patients with diabetes,accounting for 8.96%;25 patients with other diseases,accounting for 7.46%.A total of 242 patients were tubular adenocarcinoma,accounting for 72.24%.It can be seen that tubular adenocarcinoma is a common pathological type of colorectal cancer,and the other types of cases are rare.In adenocarcinoma,the most common type of differentiation is 12 cases,accounting for 49.59% of the total number;the least type of differentiation is 26 cases,accounting for 10.73%.According to the distribution of tumor sites in 335 patients with liver metastasis from colorectal cancer,ascending colon cancer was the most common,83 cases in total,accounting for 24.77%;sigmoid colon was the second,accounting for 79 cases,accounting for 23.58%;blind colon cancer was the least,accounting for 4 cases,accounting for 1.19%.According to the distribution of the time of liver metastasis,184 cases had simultaneous liver metastasis,accounting for 54.93%;151cases had heterochronous liver metastasis,accounting for 45.07%;(2)In this study,335 patients with liver metastasis of colorectal cancer were included,of which 72 common symptoms were collected from four diagnoses of traditional Chinese medicine,including tongue diagnosis and pulse diagnosis information.Among 335 patients with liver metastasis of colorectal cancer,the 10 symptoms with the highest frequency were: 141 cases of loose stools,accounting for 42.09% of the total;135 cases of anorexia,accounting for 40.30% of the total;abdomen There were 114 cases of distention,accounting for34.03%;88 cases of defecation,accounting for 26.27%;83 cases of abdominal pain,accounting for 24.78%;82 cases of stool frequency,accounting for 24.48%;81 cases of hypochondriac distention,accounting for 24.18%;38 cases of fullness,accounting for23.28%;68 cases of fatigue,accounting for 20.30%;66 cases of spontaneous sweating,accounting for 19.70%.Among the tongue and pulse information,the top five tongue images are: 136 cases with greasy fur,accounting for 40.60%;126 cases with red tongue,accounting for 37.61%;123 cases with white fur,accounting for 36.72%;111 cases with thin fur,accounting for 33.13%;79 cases with tongue and tooth marks,accounting for23.58%;107 cases with stringed pulse,accounting for 31.94%;91 cases with deep pulse,accounting for 27.16%;82 cases with weak pulse,accounting for 24.48%;65 cases with slow pulse,accounting for 19.40%,pulse weakness 60 cases,accounting for 17.91%.According to the cluster analysis,the most common types of colorectal cancer are spleen deficiency,spleen kidney yang deficiency,liver kidney yin deficiency,liver depression and qi stagnation,and dampness heat stasis.There are 104 cases of spleen and kidney yang deficiency syndrome,accounting for 31% of the total cases;96 cases of spleen and kidney yang deficiency syndrome,accounting for 28.6% of the total cases;41 cases of liver and kidney yin deficiency syndrome,accounting for 12.2% of the total cases;44 cases of liver stagnation and qi stagnation syndrome,accounting for 13.1% of the total cases;50cases of damp heat and stasis syndrome,accounting for 14.9% of the total cases.(3)The distribution of TCM syndromes was correlated with age,smoking and drinking history,tumor differentiation and tumor location(P < 0.05),but not with pathological type and the time of liver metastasis(P > 0.05).Conclusion:(1)General situation distribution: 335 patients with liver metastasis from colorectal cancer were included in this study,including 193 males,accounting for57.61% of the total,144 females,accounting for 42.99% of the total.The ratio of males to females is 1.34:1.The number of males is more than that of females.Among 335 subjects,the proportion of patients younger than 30 years old was the least(1.49%),and the proportion of patients aged 61-70 years old was the most(25.97%).(2)Personal distribution: there were 56 patients with bad habits such as smoking and drinking in the past,accounting for 16.72%;the patients without smoking and drinking history accounted for a large proportion,accounting for 83.28%.There are 81 patients with other combined diseases,18 patients with history of coronary heart disease,accounting for 5.35%;8patients with digestive tract disease,accounting for 2.39%;30 patients with diabetes,accounting for 8.96%;25 patients with other diseases,accounting for 7.46%.(3)Distribution of disease-related conditions: of 335 patients with liver metastasis from colorectal cancer,242 were tubular adenocarcinoma,accounting for 72.24%.It can be seen that tubular adenocarcinoma is a common pathological type of colorectal cancer,while the rest are rare.The degree of differentiation was the most common in 120 cases,accounting for 49.59% of the total;the number of low differentiation type was the least,accounting for26 cases,accounting for 10.73%.According to the distribution of tumor sites,ascending colon cancer is the most common,with 83 cases in total,accounting for 24.77%;sigmoid colon cancer is the second,with 79 cases in total,accounting for 23.58%;blind colon cancer is the least,with 4 cases in total,accounting for 1.19%.According to the time distribution of liver metastasis,there were 184 cases of simultaneous liver metastasis,accounting for 54.93%;151 cases of heterochronous liver metastasis,accounting for 45.07%;190 cases with diameter ≤ 3cm,accounting for 56.72%;145 cases with diameter > 3cm,accounting for 43.82%.The number of single focus was 202 cases,accounting for 60.3%;the number of multiple focus was 133 cases,accounting for 49.7cases,243 cases had surgery,accounting for 72.54%;275 cases had chemotherapy,accounting for 82.09%;79 cases had intervention,accounting for 23.58%.
