| Objective:By using gout Tongluo agent combined with diclofenac sodium double release enteric capsule to treat damp-heat accumulation knot type acute gouty arthritis,observe the changes of clinical symptoms and signs,TCM syndrome score,joint symptom score,laboratory index(esr?crp?ua)before and after treatment,objectively evaluate the clinical efficacy and safety of gout Tongluo agent combined with diclofenac sodium double release enteric capsule in treating damp-heat accumulation knot type acute gouty arthritis,and provide clinical basis and experimental basis for Tongluo agent treatment of this disease.Methods:60 patients with acute gouty arthritis diagnosed as damp-heat accumulation type were randomly divided into treatment group(30 cases)and control group(30 cases).The control group was given oral diclofenac sodium double release enteric capsule,and the treatment group was given gout Tongluo agent on the basis of the control group.The comparison of joint symptoms(joint pain vas score,joint tenderness score,joint swelling score,limited joint activity score),TCM syndrome score,laboratory index(esr?crp?ua)and safety index were recorded before treatment and 14 days after treatment.Results:In the course of this study,60 patients completed this study.(1)Baseline comparison: there was no statistical difference(p>0.05)between the two groups before treatment in terms of sex,age,complications,course of disease,TCM syndrome score,clinical signs,laboratory indicators,and safety indicators.(2)Comparison of curative effect: the effective rate of the treatment group was 90%,and the effective rate of the control group was 70%.After chi-square test,the p< was 0.05,which was statistically significant,and the effect of the treatment group was significantly higher than that of the control group.(3)Joint symptoms(joint pain vas score,joint tenderness score,joint swelling score,joint movement restriction score)were compared: all the signs in the two groups were significantly improved(p<0.05)after treatment,and the improvement in the treatment group was better than that in the control group(p<0.05).(4)TCM syndrome score comparison: after treatment,the patients in the treatment group had no significant adverse joint flexion and extension(p>0.05),the other TCM syndromes after treatment compared with before treatment,symptoms significantly improved(p<0.05),TCM syndromes score was lower than before;control group patients had no significant adverse joint flexion and extension(p>0.05),and other TCM syndromes after treatment compared with before treatment,symptoms significantly improved(p<0.05).The improvement of joint fever,joint pain,swelling and unfavorable flexion and extension in the treatment group was significantly better than that in the control group(p<0.05),the improvement degree of thirst was not statistically different from that in the control group(pp<0.05),and the TCM syndrome score was significantly lower than that in the control group(p<0.05).(5)laboratory index(esr?cr?ua)comparison: each index of the two groups after treatment was lower(p<0.05)than before treatment,and the treatment group was better than the control group(p<0.05).(6)Safety evaluation: the blood routine,urine routine,stool routine,liver and kidney function of the two groups were not abnormal during the whole study,and one case of mild gastrointestinal discomfort was found in both groups.After symptomatic treatment,the administration continued without other adverse reactions,indicating that the drug use in the treatment group and the control group was safe during the whole study.Conclusion:Gout Tongluo agent combined with diclofenac sodium double release enteric capsule can not only significantly improve the symptoms of joint pain,but also can effectively reduce the esr?crp ?ua level,improve the clinical symptoms and signs of traditional Chinese medicine,alleviate the pain of patients,and good safety,which is worthy of clinical promotion. |
