ObjectiveAnalyze the treatment effect which is better between the operation of releasing lateral retinacular and the operationg of releasing lateral retinacular and repairing medial and the operationg of tibial nodule displacement for curing patellar subluxation,to provide a meaningful refence for department of orthopedics doctors to choice method of operation.MethodsThis study retrospectively analyzed and analyzed in our hospital from January 01,2014 to June 30,2017 in our hospital surgical treatment of patellar subluxation in 62 patients(78 knees),the age range of 14-45 years old(Average 20.1 years old).Among them,28 knees(22 cases)with lateral lobe support were set as group A,26 knees(20 cases)with lateral loosening and tight knuckles as group B,24 knees Example)set to C group.Three groups of patients with knee using the same treatment: anesthesia, antibiotic types and usage, anticoagulant therapy, analgesic symptomatic treatment, local ice, postoperative care, brace and functional exercise. The patellofemoral commissure angle, patellofemoral index,Lysholm score and IKDC score of all patients were obtained preoperatively, and there was no statistical difference between the three groups. All the patients in group A,B and C were measured and calculated the appropriate angle of patellofemoral and patellofemoral index by the same method. The Lysholm score scale and the IKDC2000 subjective knee score scale were used to assess the patellofemoral joint function in the three groups after surgery.Three groups of postoperative indicators of statistical comparison using spss system to complete: the test was used to compare the three groups of patellofemoral angle, patellofemoral index comparison;three groups of postoperative Lysholm score and IKDC2000 score is good grade data, the use of rank sum Test for comparison,P<0.05 for the difference was statistically significant.ResultsPostoperative 78 knees(62 patients)were discharged after surgery were satisfactory complete follow-up results. The fit angle of patellofemoral plexus, patellofemoral index,Lysholm score and IKDC score were similar between the three groups, with no significant difference(P> 0.05).There were no pain discomfort or knee swelling at 78 knees after discharge. No recurrence of subluxation occurred in all three knees. All the three procedures were significantly improved to improve the stability of the patellofemoral joint and restore the patella motion to normal. There was no significant difference in the appropriate angle of patellofemoral and patellofemoral index between groups A,B and C(P> 0.05).There was no significant difference between the two groups(P> 0.05).IKDC score excellent overall similar rates, the difference was not statistically significant(P> 0.05).ConclusionThe study shows that the lateral support band release surgery, the external loose internal fixation and tibial tubercle translocation of three surgical procedures have achieved good results, all the patellofemoral joint stability has been significantly improved, all the patella Movement track returned to normal, can significantly eliminate the symptoms of knee pain, significantly improve the quality of life of patients, the clinical results have been affirmed. For postoperative patella stability, knee function, there was no significant difference between the three surgical patients.
