ObjectiveThrough the calculation and statistics of perioperative blood loss of total hip arthroplasty(THA)in patients with femoral head necrosis,the influencing factors of perioperative recessive blood loss in patients undergoing total hip arthroplasty were analyzed.Mechanisms to evaluate more comprehensively and systematically the patients undergoing total hip arthroplasty for femoral head necrosis,and to provide appropriate interventions to reduce the risk of perioperative high blood loss to reduce perioperative anemia and related complications.Accelerate the patient's rapid recovery.MethodsAccording to the inclusion exclusion criteria,the clinical data of 76 patients with ONHH who underwent total hip arthroplasty at the femoral head specialist of Linfen City People's Hospital were collected from 2016.12 to 2017.12.The basic characteristics of the patient were recorded:history,age,gender,weight,height,femoral head Degree of necrosis(ARCO staging based on magnetic resonance imaging results).According to gender,age(bounded at 60 years of age),BMI(body mass index BMI<28 kg/m~2 in non-obese group,BMI?28 kg/m~2 in obese group),history(trauma,hormones,alcohol,and idiopathic factors),The operative time(60 min)and the degree of femoral head necrosis(according to magnetic resonance imaging results)and other criteria were grouped.For patients undergoing elective total hip arthroplasty(all primary replacement),the entire procedure was performed by the same physician team and performed by the same surgeon,and all patients were treated with a lateral approach.The hemoglobin value,hematocrit,intraoperative blood loss,blood transfusion,postoperative drainage,and operation time were recorded before and after surgery.According to the calculation equation of the equation of Gross equation,the blood volume of the patient is first calculated,and then the total hemorrhage is calculated according to the difference of the preoperative and postoperative hematocrit(Hct);the difference between the total blood loss and the dominant blood loss is the implicit blood loss.The amount of dominant blood loss included intraoperative blood loss,postoperative drainage,and the amount of exudate from the surgical incision after surgery.Statistical analysis was performed using statistical software SPSS21.0.The comparison of different influencing factors(age,gender,BMI,medical history,imaging staging,and operative time)was performed using a univariate analysis.In the univariate analysis,two samples were compared using two Independent sample t-test,the comparison of multivariate variables was analyzed by one-way variance analysis.After data analysis,the variable P?0.10 was considered as the potential impact variable,and the multiple linear regression analysis(taking the forced entry method)was used to evaluate the impact of implicit Risk factors for sexual blood loss.P<0.10 was considered statistically significant.ResultAfter statistics and analysis:1.Total hip arthroplasty in patients with femoral head necrosis total blood loss was 1067±309mL,recessive blood loss was about 33.1%of total blood loss,354±221mL.2.Age,long-term drinking and long-term use of steroids,body mass index(BMI)are the main influencing factors for perioperative occult blood loss in total hip arthroplasty in patients with femoral head necrosis.ConclusionAge,long-term heavy drinking,hormone taking history and body mass index were the influencing factors of recessive hemorrhage in total hip arthroplasty in patients with femoral head necrosis.
