Objective: To compare the results of one-stage bilateral total hip arthroplasty (THA) and staged bilateral THA in patients with avascular necrosis of the femoral head, and to determine the safety and efficacy of one-stage simultaneous bilateral THA.Methods: From January 2000 to September 2007, 92 patients from the Orthopedic Spinal Surgery and Orthopedic Traumatology Department ,with bilateral avascular necrosis of the femoral head, had total hip arthroplasty . These patients were divided into 2 groups. Group A included 54 patients (108 hip joints) who had one-staged bilateral THA, and Group B included 38 patients who had staged bilateral THA. The operating time, the amount of blood transfused intra and postoperatively, the total hospitalization fee and postoperative complications were compared between the 2 groups.Results: There was no significant difference in operation time, the amount of blood transfused intra and postoperatively, and the rate of post operative complications between group A and group B. However the hospitalization time and hospital fee was significant difference between the two groups. The recovery of function combined with other aspect indicates one -staged bilateral total hip arthroplasty is superior to staged bilateral hip arthroplasty. Conclusion: One-stage simultaneous bilateral total hip arthroplasty is a safe and efficacious procedure for patients with avascular necrosis of the femoral head.
