Analysis Of Curative Effect Of Total Hip Arthroplasty For Late Osteonecrosis Of The Femoral Head

Objective:Analysis of related factors influencing total hip arthroplasty in treatment of avascular necrosis of the femoral head effect,Evaluate of our hospital clinical efficacy of the treatmentl.Method:In our hospital from 2012 to 2013 for total hip arthroplasty for late osteonecrosis of the femoral head(Steinberg III b- stage IV) a total of 213 patients(218 hips) were followed up,finally in 208 cases(211 hips) were followed up,according to etiological factor is divided into 74 cases of traumatic,alcohol in 44 cases,60 cases of hormone,special of 30 cases with prosthesis;to implant fixation type is divided into biological type in 146 cases,62 cases of mixed type.To implant fixation type is divided into biological group and mixed group,according to the comparison of patients with preoperative and postoperative Harris score,operation effect analysis;describe the operation complications occur,analyzes its influence on the operation effect..Results:In 208 cases(211 hips) follow-up results:biological group average operation age58.43 ±7.69 years old,mixed group of 67.17 ± 8.54 years,two groups have significant difference.Harris score from preoperative 37.25 points increased to 89.38,including excellent(90 points or more) in 56 cases(45.78%),good(80-89) in 78 cases(64.39%),qualified(70-79) in 24 cases(23.43%),the excellent and good rate was 92.28%.In January,1 years after the operation of biological group Harris score(81.27± 10.43) /(89.57± 7.83),mixed group(79.25± 10.79) /(88.36 ± 10.25),P values were greater than0.05,no significant difference between two groups.The biological group operation time was98 + 25 minutes,mixed group operation time of 113 ± 31 minutes,there were significant differences in the amount of bleeding during operation;biological group(417 ±75) ml,the amount of bleeding in the operation of mixed group(398 ± 85) ml, no significant difference between two groups;biological group postoperative drainage(98 ± 12 ml mixed group),postoperative drainage volume(72 ± 13) ml,the two groups have statistical difference.Complications:3 cases of deep venous thrombosis of lower extremity(1.44%),4cases of nerve injury(1.92%),2 cases of heterotopic ossification(0.96%),no infection,prosthesis loosening and dislocation of joint.Conclusion:1.According to the results of patient imaging and general,to select of biological prosthesis or bone cement prosthesis.2.No differences in short-term curative effect of biological prosthesis and bone cement prosthesis in the treatment of advanced avascular necrosis of femoral head.3.Total hip arthroplasty for late osteonecrosis of the femoral head in our hospital obtained satisfactory curative effect.4.Evaluation of long-term efficacy of total hip arthroplasty for the treatment of femoral head necrosis needs long time,the more system fl.up.
